📣 Now Hiring: Registered Nurse – Rounding / Case Management
Are you an RN who enjoys working directly with hospital patients, coordinating care transitions, and making a real impact? Join our team at Pathways Health Partners and play a vital role in patient-care rounding and discharge planning.

What You’ll Do:
You’ll visit patients at the bedside, assess their needs (physical, psychosocial, functional), collaborate with the care team, and help ensure safe transitions from hospital to home or other settings. You’ll identify patient barriers to care, coordinate resources, and support the patient’s discharge plan.

What We Require:
• A valid Florida RN license
• Preferably 1-2 years of RN experience and care-management or hospital experience
• Strong assessment, communication and documentation skills
• Ability to work independently and as part of an interdisciplinary care team
