Leland Medical Clinic Leland Medical Clinic, supported by Delta Health Alliance, is a NCQA Recognized Patient Center

Leland Medical Clinic, owned and fully operated by Delta Health Alliance since 2013, is a NCQA Recognized Patient Centered Medical Home (PCMH) and a Rural Health Clinic offering area patients a team approach to primary healthcare. The PCMH approach means that the entire LMC staff, as well as the patient, work together as a team by focusing on all aspects of the patient’s health using enhanced technology and chronic disease management along with behavioral health management as needed. We use Athena Health as our electronic health record to enhance delivery of care by allowing our providers to spend more time examining the patient and less time on record keeping. The LMC staff can connect electronically to pharmacies and area hospitals and patients have real time access to their medical records via our online patient portal. Services include management of chronic and acute illness, Immunizations, pediatric care, pre-employment or school physicals, on-site lab, treatment of minor injuries, sports physicals, wellness exams/EPSDT screenings, ECGs, injections, Digital X-Ray, Mammograms, counseling, medication management, case management and nutrition education. We’re proud to spotlight Teresa Womack, RN, Director of Primary Care at Leland Medical Clinic. With nearly 25 years of nursing experience, Teresa brings dedication, compassion, and leadership to everything she does.

In honor of American Heart Month, Teresa is encouraging our community to stay proactive about their health by scheduling annual wellness checks. At Leland Medical Clinic, these visits offer EKGs, access to a nutritionist, and additional support through the Delta Wellness Center to help support heart health.

We are so appreciative of all that Teresa does for Leland Medical Clinic and for the care she provides to our patients and community ❤️

EMDR therapy is an evidence-based approach that helps the brain safely reprocess traumatic or distressing memories so they no longer hold the same emotional weight. Using guided eye movements or rhythmic stimulation, EMDR supports healing from trauma, anxiety, and deeply rooted negative beliefs.
