Monday Moments at Five Counties 💛

Today we’re saying adios (for now!) to our amazing nurse Haelee as she heads out on maternity leave! 👶✨ We are so excited for this next chapter for her and can’t wait to meet that sweet baby soon.

Our hearts are full celebrating her today — and a big thank you to Susan Gunn for treating us all to delicious donuts 🍩

Haelee, you will be missed, but your Five Counties family is cheering you on every step of the way! 💐💕

Now it’s time to switch gears and get ready for our Auxiliary Luncheon — a busy and beautiful Monday here! 💛

Just another fun Friday at Five Counties! 🎲🧩🍦

We kicked things off with some delicious root beer ice cream (trust us… they didn’t last long enough for pictures 😄🍦), and then rolled right into an afternoon full of games and laughs!

From Rummikub and Yahtzee to working on puzzles together, our residents kept the fun going and the competition friendly. 💛

It’s the simple moments like these—good treats, great company, and lots of laughter—that make Fridays pretty special around here.

Five Counties is honored to partner with ICAP to bring the Heart-to-Home: ICAP Senior Meals on Wheels program to our area starting May 4th! 🍽️❤️

This program is another step in strengthening the support available to seniors in our community—ensuring more individuals have access to nutritious meals, friendly visits, and the ability to remain safe and independent at home. We are grateful to be part of a community that values coming together to care for one another.

✨ A Night to Remember at Five Counties ✨

We had the absolute pleasure of welcoming our local high school prom students for their Grand March, and what a special afternoon it was!

From elegant gowns to sharp suits, the students looked absolutely amazing. Our residents loved every moment—smiles, laughter, and excitement filled the room and made it a truly memorable experience for all. ❤️

A very special congratulations to our very own Prom King and Queen, Irving Jordan and Betty Tomac! 👑 What a fun and meaningful way to celebrate alongside the students.

Moments like these are so meaningful, bringing generations together and creating memories that will last a lifetime.

Thank you to the students, families, and school for including us in such a special tradition. We hope you all had an incredible prom night! 🎉✨

🍽️❤️ Something exciting is coming to Five Counties! ❤️🍽️

We are thrilled to announce that Meals on Wheels will be available soon, starting in May!

ICAP’s 60’s Plus Dining, has recently expanded services to the 5-county area in Western South Dakota. ICAP is a community-based non-profit organization that supports seniors and individuals in need, and we are honored to partner with them to help bring this service to our community.

At Five Counties Nursing Home, caring for our community and our elderly population has always been at the heart of what we do. This program is designed to support elderly individuals who may need assistance with meals, helping them maintain independence, health, and connection.

🍲 Warm, nutritious meals delivered right to your door
🤝 Financial assistance will be available for those who qualify

Whether it’s for yourself, a loved one, or someone in the community, we can’t wait to serve you in this new way.
