Classic silent sinus of right maxillary sinus. This condition occurs from blockage of the sinus resulting in negative pressure and lowering of the inferior orbital wall causing facial pain, pressure and in severe cases, enopthalmos (lowering of the eye) leading to vision changes.

Treatment is typically surgical and requires removal of the uncinate process which is typically plastered to the lateral orbital wall. Since the natural opening is low in these patients, it’s crucial to stay inferior to avoid entering into the orbit. Often times, as in this case, 70 degree angled scopes are required for proper visualization, making them technically challenging.

Example below performed in my office under local anesthesia. This patient had no pain and was out of the office in 30 minutes.

Classic bilateral co**ha bullosa of the middle turbinates contributing to nasal congestion and sinus outflow obstruction. While typically asymptomatic, these anatomical variants can contribute to both chronic and recurrent sinus symptoms. Along with opening the sinus, they can be resected to improve airflow and sinus drainage. Often performed in an operating room under general anesthesia, I perform this procedure in the office under local anesthesia, typically only taking 20 minutes. This approach significantly reduced patient recovery time and cost.

According to Mayo Clinic, when an airplane climbs or descends, the air pressure changes rapidly. The eustachian tube often can't react fast enough, which causes the symptoms of airplane ear. Swallowing or yawning opens the eustachian tube and allows the middle ear to get more air, equalizing the air pressure.
