For everyone's safety, in response to COVID-19, we have uploaded all our patient forms on the website at https://greatlakesdental.net/patient-forms/. These forms can be filled at home via a computer / laptop only (not cell phone).

After you have filled all sections, please email forms to us at info@greatlakesdental.net with your name in subject line so we can save it in your file folder. If you have any questions, please call us 847 984 2548 or TEXT 224 548 5567 on Tuesday, Thursday, Friday 10-6 pm and Saturday 9-1pm.

Our office is OPEN Tuesdays 10 am to 6 pm, Fridays 10 am to 6 pm, and two Saturdays a month 9 am to 2 pm. Routine and emergency dental care procedures are being performed. As always, we adhere to universal precautions with every patient visit. How to Protect Yourself from COVID-19 Related Scams
Several different scams are being reported by both the Federal Trade Commission (FTC) and the World Health Organization (WHO) that we’d like for you to be aware of. The goal of these scams is to acquire personal identification and financial information for fraudulent access to financial accounts. These scams are being tailored toward both the anticipated government stimulus program that will provide many Americans with payments as well as virus-related information.

The best defense against any type of scam is education and awareness. Please take a minute to review the helpful reminders below and keep yourself protected:

The government will not ask you to pay anything to receive funds from their anticipated stimulus plan. There will not be fees or charges associated.

The government already has the information they need to process any payments. They will not request your social security number, bank account information, or credit card number.

The government stimulus/payment program has not yet been implemented. If you receive any communication via email, phone or direct mail that you can receive this money now – this information is being sent to you from a scammer.
Be aware of fraudulent emails claiming to be from a government organization. If you receive an email claiming to be from the WHO or CDC, don’t click on any links provided. Instead, go directly to the respective website through a separate search engine to verify information.

Do not rush to donate to charities or crowdfunding sites. Please do your homework and research charities and organizations to verify validity and never provide a donation by cash, gift card, or wire.

Do not click on any links from sources you do not know. Clicking fraudulent links can download viruses to your computer or device.

Ignore online offers for vaccinations. There currently are no vaccines, pills, lotions, prescriptions or over-the-counter medications or products available to treat or cure the COVID-19 virus.

To the patients families of Great Lakes Dental Solutions,

The health and safety of our patients and team members is our top priority. This is a critical moment in our world, and things are unfolding at a rapid rate as it relates to the COVID-19 virus.

In light of the current COVID-19 crisis, we have decided that the most prudent action to take is to close the office starting Tuesday March 17, and continuing until the end of this week. We will make a decision at week’s end regarding any possible closures moving forward.

Per updates from the CDC, we have approached the period where social distancing is critical to help reduce the transmission of COVID-19 virus.

We will be monitoring the crisis daily and make changes based upon recommendations from experts.

Dr. Soni will handle any emergencies that should arise. Please call the office to speak to a staff member during our reduced hours 11am-1pm or, if after hours. Leave a message or send a text to (224)548-5567 and we will attend to the situation as soon as possible. We will be rescheduling all exams, cleanings, and fillings – non essential procedures.

We apologize for any inconvenience and appreciate your understanding as we are choosing to close the office out of abundance of caution to keep everyone safe and flatten the curve. This is an important time for all of us to pull together.
