Legacy Hospice (established in 2020, licensed 2021) is a community-based hospice provider offering services that optimize comfort &dignity to meet the physical, emotional, psychological, practical, and spiritual needs of the patient along with their family. Legacy Hospice is dedicated to nurturing our patients that are nearing the end of life by providing quality care within the security and comfort of their home. We focus on promoting integrity of the body, mind, and soul as the individual works through life’s last development process. Our support includes Nursing, Hospice Aide, Physician, Social, Spiritual Care, Bereavement, Therapy & Volunteer Services.

Congratulations to Evon Erickson, the winner of our Mobility Basket Giveaway at the recent Primrose Assisted Living Health Expo! It was a pleasure meeting so many wonderful residents and staff while sharing information about the compassionate care and support services offered by Legacy Hospice. We are grateful for the opportunity to connect with the Primrose community and to help spread awareness about the resources available to patients and families.

We are proud to recognize our chaplain Mark for a very special honor he recently received.

Mark was presented with a challenge coin from the Allen County Sheriff’s Office Motorcycle Unit in recognition of the many funeral services he has faithfully provided for their members and the strong partnership he has helped build between their unit and our team.

This recognition reflects the compassion, dedication, and presence Mark provides to families and first responders during some of life’s most difficult moments.

At Legacy Hospice, we believe our care does not stop at the bedside. Our mission is to walk with patients and families all the way through the journey, providing compassionate end of life care and continuing to support loved ones through specialized bereavement services long after their loss.

We are grateful for the partnership we share with the Allen County Sheriff’s Office Motorcycle Unit and for the opportunity to serve our community together.

💙 Spreading Warmth and Hope 💙

Today, our HCF team had the privilege of donating blankets to oncology offices for patients currently undergoing chemotherapy.
Small comforts can make a meaningful difference during treatment, and these blankets are a simple way to offer warmth, care, and support during a very difficult time.
To every patient fighting this battle: you are not alone. We are honored to stand beside you and your care teams throughout this journey.
A heartfelt thank you to the facilities and staff who helped make this possible!

It’s a specialized level of care focused on comfort, symptom management, and dignity when a cure is no longer the goal. Hospice patients continue to receive medications, treatments, and therapies—specifically those that provide comfort and enhance quality of life. 💜
