Position Overview: Senior Placement Specialist
Company: Modern Senior Transitions
Location: Nebraska (primarily Lincoln, Omaha, and surrounding areas)
Status: Independent Contractor (1099)
Qualifications:
Compassionate nature and strong interpersonal skills – must genuinely enjoy working with seniors and their families
Knowledge of senior care options – understanding of the differences between assisted living, memory care, skilled nursing, and long-term care
Experience in healthcare, social work, case management, or senior services (preferred but not required)
Excellent communication and listening skills
Problem-solving mindset – must be able to quickly assess needs and recommend tailored solutions
Organizational skills – ability to manage multiple client cases at once
Valid driver’s license and reliable transportation – in-person visits and facility tours are part of the role
Tech-savvy – comfortable using CRM software, email, texting, and online research tools
Background check required
Day-to-Day Responsibilities:
Client Intake & Consultation
Receive referrals from families, hospitals, rehab centers, or other professionals
Conduct initial phone call or in-person consultation to understand care needs, budget, location preference, and timeline
Research & Recommendations
Evaluate available care communities based on the client’s criteria
Prepare a shortlist of qualified options (including virtual tours, photos, and price breakdowns)
Facility Coordination
Schedule and accompany clients or family members on tours of selected communities
Communicate directly with community intake staff and marketing directors to confirm availability and service match
Advocacy & Support
Act as a liaison during the decision-making process
Help review contracts, discuss financing options (VA benefits, Medicaid waivers, etc.), and ensure a smooth transition
Follow-Up & Case Management
Check in with clients after placement to ensure satisfaction
Provide additional resources (moving services, home downsizing, etc.)
Business Development
Build and maintain relationships with care providers, hospital discharge teams, and other referral partners
Attend community events, senior expos, and networking functions to promote Modern Senior Transitions
Compensation Structure:
This is a 100% commission-based role — there is no hourly wage or base salary
Compensation is earned per successful placement
A performance-based bonus structure is available for high-performing contractors
You will be classified as a 1099 Independent Contractor, responsible for managing your own taxes and expenses
Realistic annual earnings range: $60,000 to $150,000+, depending on placement volume, referral relationships, and personal drive
Typical Work Schedule:
Monday to Friday – 8:30 AM to 5:00 PM (with some flexibility depending on client needs)
Evenings & Weekends – Occasionally required for client tours or urgent placements
Hybrid Role – Combination of office work, remote consultations, and on-site facility visits
