Caregiver Responsibilities

Sunlight has a great group of caregivers, and we are looking for a few more people to add to our exceptional team. No experience necessary — we’ll train the right person.
“It’s like taking care of a family member,” one Sunlight caregiver said. Another Sunlight caregiver said it’s a beautiful job. “If you like what you’re doing, you will do it with love,” they said.
Duties include:
1. Know, understand and follow the set care plan for the client which can include some or all the following:
• Assisting with bathing, grooming, toileting and incontinence issues
• Providing companionship and conversation
• Providing stabilization and assistance with walking
• Preparing meals and cleaning up meal-related items
• Providing medication reminders and appointment reminders
• Light housekeeping
• Accompany client to doctor’s appointments
2. Observe changes in behavior or medical condition and report changes immediately
3. Clock in and out on time using the ClearCare Telephony system
4. Complete Daily Care Log tasks
5. Daily record notes in Client Care Plan Binder and report any significant change in clients’ health/condition and communicate the same to the supervisor
6. Respond quickly to emergency situations
7. Follow all Sunlight Support guidelines, policies and procedures
Experience/Knowledge/Personality:
1. Minimum one year of experience preferred
2. High school diploma or GED
3. Familiarity with medical terminology helpful
4. Excellent communication skills and ability to provide high level of customer service in a fast-paced environment

TEAM LEAD PURPOSE:
In keeping with our vision and strategy of hiring and retaining the highest quality people to provide our client services, the Team Lead role is pivotal in maintaining enthusiasm throughout the hiring, on-boarding and ongoing training process. The Team Lead creates the first impression for caregivers and empowers and encourages them along their journey through educational and hands-on training, positive communication and support.

The Team Lead plays a vital role in protecting the company's image and mission not only to the public, but to caregivers as well. Responsible for all aspects of caregiver training, problem- solving, on-boarding and compliance.

Also, responsible for independently learning all aspects of new company procedures and training as approved and directed by the executive team.

There are no typical hours of work for this position as training is as needed.

The trainer must participate in the on-call rotation by being available to cover open shifts while on call.

To ensure client contact, the care trainer must fulfil 20 billable hours working with current clients and/ or introducing new clients to Sunlight.

Job Description Role and Responsibilities:

ORIENTATION AND ON-BOARDING:

The Team Lead will participate in orientation and on-boarding of new caregivers and provide the new caregivers a comfortable and enthusiastic environment to learn while providing key information about the company, clients and Sunlight work processes.
Evaluate a best fit, set expectations and share Sunlight’s plan for training, support and guidance for the caregiver’s future success.
Schedule new caregiver’s training shifts and brief caregivers on clients’ specific orientation.

ON-GOING TRAINING:

Ensure all caregivers have hands-on trainings with clients.
Design and document all client’s training checklists.
Ensure caregiver complete assigned Relias online trainings on time while communicating effectively and efficiently with care managers and caregivers for attendance and kudos for completion.

CAREGIVER RETENTION:

Contact ALL new caregivers 1 time per week for the first two weeks after the new caregiver begins. The first 90 days are pivotal for caregiver retention with an emphasis on the beginning 30 days. Caregivers who have a strong start and a good initial experience will be more willing to flow through some inevitable ups and downs

TEAM/OFFICE SUPPORT:

The Team Lead is in a key role in supporting the operations of the office in several capacities. These capacities include but are not limited to: Covering the Care Manager and coordinator position during times of need and filling in as a scheduled caregiver in the event of no other recourse availability and doing so works within the Care Trainer’s existing schedule of duties.

MEETINGS:

Team Lead will participate in all scheduling and team meetings and brief on the training schedule for each client.
Participate in the monthly caregivers meeting

Care Manager Purpose

The Care Coordinator is responsible for supervising and guiding the care team insuring the team is well informed well trained and supported in their day to day duties. The care manager is tasked with client care planning and data collection associated with Sunlight Senior Care.

The Care Coordinator plays a vital role in protecting the company's image and mission not only to the public, but to caregivers as well. Responsible for all aspects of caregiver monitoring and data recording, problem solving, on-boarding and compliance.

Also, responsible for independently learning all aspects of new company procedures and training as approved and directed by the executive team.

The Care Coordinator must assure the care team is complete and competent by continuously hiring, training and supervising the members of the team.

Typical hours of work with include Monday-Friday 9:00am-5:00pm and as needed based on scheduling conflicts and shift left open.

To ensure client contact, the care manager may be required to fullfill on shift hours as needed.

Care Coordinator Must participate in the on-call rotation.

ROLE AND RESPONSIBILITIES

Assure the care team is fulfilling all recruiting, scheduling and training needs as assigned.
Utilize computerized data collection system to accurately monitor, correct and report data on a timely basis.
Assure that each day’s visits are properly documented.
Be responsible for maintaining data system so records are accurate and complete.
Assure that updates are input daily as submitted by caregivers and management team.
Identify conflicts in patient care and make judgement calls as to the best path forward
Ensure all involved are up to date on the latest Care plans
Identify problems ahead of time and initiate appropriate actions
Keep up to date and detailed records on all Client cares and schedules
Coordinate with Care coordinator and training coordinator to assure quality care
Communicate appropriately with caregiver staff: Operations, Sales/Marketing, Clients and other contacts and accurately relay messages as needed.
Answers team member questions, helps with team member problems, and oversees team member work for quality and guideline compliance.
Communicates deadlines and client goals to caregiver team members
Develops strategies to promote team member adherence to company regulations and performance goals.
Conducts team meetings to update members on best practices and continuing expectations.
Generates and shares comprehensive and detailed reports about team performance, mission- related objectives, and deadlines.
Ensures company brand materials and physical working spaces meet and exceed company presentation standards.
Provides quality customer service, including interacting with caregivers, answering caregiver enquiries, and effectively handling caregiver complaints.
