Foundations Medical Adult Day Care and Psychiatric Rehabilitation Program

Foundations Group is the parent company of Foundations Medical Adult Day Care and Psychiatric Rehabilitation Program. Foundations is designed to meet the needs of individuals (ages 18 and older) who have co-occurring health issues including chronic illness, mental illness and substance dependence. Attending Foundations allows individuals to remain at home with their loved ones and in their community. Foundations PRP facilitates an individual's recovery, including the individual's ability to make decisions while creating opportunities regarding home, school or work and community.
