SC ACMA Who can you nominate?

🌟 Nominate a Health Care Hero. 🌟

Because even heroes need help sometimes.

Behind every successful care transition is a hero you may never see.

A social worker battling cancer while guiding families through theirs.

A case manager working double shifts—while caring for a loved one at home.

These are the heroes we honor.

Nominations are now open for the ACMA Hero Award, presented with premier partner, Xsolis—recognizing health care professionals in case management and transitions of care who continue to serve patients while facing extraordinary personal challenges.

🏆 Know someone who deserves recognition—and a little support? Nominate them today.
