Another window will pop up asking you to either log in with your username or activate your account. Click on Activate Account.
Another page will appear that needs to be filled out Completely.
Enter First and last name exactly as it appears on your activation Letter, your date of birth, and your email twice. It will also require you to choose THREE security Questions and Answers to verify your identity, when logging in occasionally.
The site ID Phrase can be anything you want it to be, this is an Extra precaution. When you log into your portal this picture should Always be displayed. If it is not the picture you have chosen, DO NOT LOG IN.
You then need to create a login name less than 12 characters and create a password. You will enter your password twice to confirm. PASSWORD MUST BE AT LEAST 7 CHARACTERS INCLUDING UPPER AND LOWER CASE LETTERS, NUMBERS, & SPECIAL CHARACTERS.
THE ACTIVATION CODE WILL BE ON THE PORTAL ACTIVATION SHEET GIVEN TO YOU AT THE OFFICE DURING YOUR APPOINTMENT, OR YOU CAN CALL THE OFFICE 716-433-26674 AND OBTAIN VERBALLY. THE ACTIVATION CODE IS CASE SENSITIVE. ONCE ENTERED, CLICK SUBMIT AT BOTTOM LEFT OF THE PAGE.
A copy of these directions with pictures can be obtained in the office.
You will now be logged into your account. From here you can see your chart, message the office, look at your medications, appointments, and save a copy of your medical record.
If you are having problems navigating within your account, please go back to the website, Click on the patient portal at the top of the page and then scroll down to the bottom. There is a video you can watch that will help explain how to navigate throughout your chart.
