Cervical spinal deformity- due to degenerative changes (arthritis and “wear and tear”), this person developed an abnormal alignment of the neck (cervical deformity). This caused compression of the spinal cord and nerves. I performed a 2 stage surgery in order to correct the alignment and take pressure off of the nerves and spinal cord. First, I performed an approach from the front (anterior), and removed 4 discs. Spacers and a plate and screws were then placed to hold the neck in proper alignment. Then, I placed screws and rods in the back of the neck for added stability. The result was a big improvement in spinal alignment.

Sacroilitis and SI Joint fusion: The SI joint is located on either side of the sacrum. The sacrum connects to the lumbar spine above and the pelvis on either side forming the Sacroiliac Joint (SI Joint). This is a very common source of back, buttock and leg pain. The SI joint is designed to move very little. When it moves too much, it can cause excruciating pain. Fusion of the joint can relieve this pain. I perform this surgery through a 3 inch incision, and place 3 dowels across the SI joint to stabilize it. This is performed without destruction of muscle. Patients go home 2 hours after the surgery is complete. I have had truly remarkable results utilizing this technique!

Chronic Subdural Hematoma- a subdural hematoma occurs when blood accumulates between the brain and its lining (the dura mater or “tough mother”). Chronic subdural hematomas occur in older individuals who suffer minor head trauma. Veins crossing from the brain to the dura mater slowly bleed, causing accumulation of blood in the subdural space. The blood slowly breaks down, and causes compression and irritation of the brain. This person had a minor fall 2 moths ago, and presented with a 2 day history of mild right sided weakness and headache. I removed the majority of the blood through a hole I created in the skull (called a “burr hole”—see 2nd pic). @ Swedish Medical Center

“The Domino Effect”- the spine is a series of joints stacked on top of one another. Fusion prevents movement at one or more of theses joints, changing the biomechanics of the spine. That puts more stress and strain on the joints next to the fusion. This pic illustrates the domino effect—the person underwent a 2 level neck fusion 5 years ago, and subsequently developed severe break down of the joint (disc) above the fusion causing instability. In order to correct the problem, I had to perform another fusion above the previous fusion. This is why I make every effort to avoid fusion in my practice in favor of artificial discs or minimally invasive decompressions.

Throwback to residency- high-powered gun shot wound to the head entering on the left side of pic and exiting on the right. High-powered guns are capable of sending a bullet through the skull in this way, causing a pressure wave of destruction through the brain tissue. Low-powered guns typically can pe*****te the skull, but are not powerful enough exit the skull. This results in the bullet ricocheting off the skull and back into the brain. Gun shot wounds to the head portend a very poor prognosis.

Large disc herniation in the cervical spine (neck) causing spinal cord compression and weakness and numbness in the patient’s arms and legs. In the time of corona virus, this certainly qualifies as urgent (not elective!), and I performed a discectomy and fusion (ACDF). There was a fracture of the spine as well, so artificial disc was not an option.
