I had such a blast at the APTA’s Combined Sections Meeting in Anaheim, CA the past three days! This is the largest annual gathering of PTs in the country, a place to gather and share information across all disciplines with over 15,000 in attendance! Some of my favorite lectures that I’m ready to integrate on Tuesday were about breath mobility, approaches for chronic pain, a deep dive into hyper mobility in pelvic rehab, and integrating the whole body into healing for the pelvis. Such great speakers! It’s so great to see the amazing ways people continue to move our profession forward and how passionate PTs are about helping our patients. Love that!

One of the first steps toward a healthy pelvic floor — whether you’re having symptoms or not — is awareness.
Many people have never actually noticed what their pelvic floor does when they try to engage it… or when they try to let it go.
If you’re sitting comfortably, you can gently explore:
• what it feels like to lightly contract
• what it feels like to release
• whether those two sensations feel distinct or not
There’s no need to force anything or “do it right.”
This isn’t about performance — it’s about noticing.

Pain doesn’t require ongoing damage to persist.

Pain is created in the nervous system — whether you cut your finger or experience pelvic pain. When the brain has learned to perceive an area as a threat, it can turn the volume up on pain signals and lower the threshold for what feels painful.

That doesn’t mean the pain is imagined.
It means the nervous system may need support and recalibration.

more squeezing isn’t the answer.

Tight muscles are usually overworked, not weak.
Stress, breath-holding, posture, and fear can all keep the pelvic floor stuck in protection mode.

Healing starts with safety, coordination, and learning how to let go — not forcing more Kegels.
