The challenge shows up when the task isn't inherently stimulating β homework, chores, waiting, anything that requires sustained effort without immediate payoff. This was something I didn't realize before I went back to school to become a clinical counselor. My youngest was so great at getting lost in her play that I thought, "Maybe she's just busy; maybe it's not ADHD." Flash-forward to a training and I'm thinking, "Yup, she does that, she does that, she does that" and then finally, HYPER-FOCUS! "Yup! Hey, I think my kid has ADHD!"
If you've been told 'they totally can focus so it can't be ADHD,' that's worth revisiting with a proper evaluation.
