Home Visit Case: The Importance of Proper Evaluation for Wounds

Recently, during one of our home visits, we were called to see a 98-year-old patient with a large hematoma filled with blood clots. The patient had previously been seen at an emergency room, but unfortunately, no diagnostic work-up or intervention was performed before being sent home.

On examination and bedside ultrasound, we confirmed the presence of a large, organized hematoma requiring drainage and careful wound management.

This case serves as an important reminder:
➡️ Any wound, swelling, or skin discoloration—especially in elderly or medically fragile patients—should always be taken seriously.
➡️ A thorough vascular and soft tissue evaluation, including ultrasound when appropriate, is essential to identify underlying bleeding, infection, or compromised circulation.
➡️ Early diagnosis and proper management can prevent complications such as infection, necrosis, or prolonged pain.

• Definition: Instead of injecting larger amounts of neurotoxin (Botox®, Dysport®, Xeomin®, etc.) deeply into muscle to fully relax it, micro-dosing uses lower doses spread out across several small, superficial, or shallow injection points.
