Horizon Recuperative Care, is a respite program providing intensive case management and social services for displaced individuals that have been hospitalized for an illness or injury and need time and resources to recuperate. Our program focuses its services on coordinated planning, care, and a clean safe, and warm environment. Our goal is to transform health services and health outcomes for vulnerable homeless individuals that require a shoulder to lean on as they strive for a better tomorrow.
