Horizon Recuperative Care, is a respite program providing intensive case management and social services for displaced individuals that have been hospitalized for an illness or injury and need time and resources to recuperate. Our program focuses its services on coordinated planning, care, and a clean safe, and warm environment. Our goal is to transform health services and health outcomes for vulnerable homeless individuals that require a shoulder to lean on as they strive for a better tomorrow. Working at Horizon Recuperative Care isn’t just a job for me, it’s a calling. Every day, I have the opportunity to be part of a team that helps people rebuild their lives during one of the most vulnerable times they may ever face. As part of the Social Services team, it means advocating for people who may feel unseen, and helping them see that they still have a future worth fighting for. I am privileged to see firsthand how healing is about more than medical recovery; it’s also about dignity, stability, and human connection.

🌟 Employee Recognition Spotlight 🌟

We’re so proud to celebrate Evelyn Lopez, Medical Coordinator at Horizon Recuperative Care. Recently, a social worker from Adventist Health White Memorial recognized Evelyn for going above and beyond when helping patients transition from the hospital to our community. From dialysis and primary care appointments to case management follow-ups, Evelyn makes sure no detail is missed, supporting recovery, preventing readmissions, and treating every patient with dignity and respect. Her colleagues see it every day too: professionalism, kindness, and a true heart for the people she serves. Evelyn reminds us all what compassionate care looks like in action.
