Our program is nationally recognized for assisting folks who are returning home from incarceration.

If someone has ever been judicially impacted, our team of community health workers are ready to assist from medi-cal/medi-care to probation and parole.

Hello people's St. John's Well Child & Family Center, now called St. John's Community Health recently lost one of it's team but the program and it's team members continue to work hard to assist community members. Justice impacted and formally incarcerated come to see the team.

The RISE team is at the Los Angeles County Reentry Resources Fair today. Stop by at the Alameda Parole Complex from 9:00am-2:00pm to be connected with your California ID, Medi-Cal, CalFresh, employment and education links, and other support services!
