Adam Geach D.M.D., M.D.Sc www.geachdental.com

Dr. Adam Geach is a Board Certified dental specialist in Prosthodontics. Prosthodontists undergo 3 years of formal residency training beyond dental school, specializing in dental cosmetics, implants and complex reconstructive dentistry. Our cases range from simple fillings, bondings, or veneers (cosmetic procedures) to complex, full-mouth reconstructions requiring replacement or restoration of all teeth. Our primary goal is to provide you with the most natural appearing, well fitting dental restorations possible. We are conservative in our approach and you will never be sold unnecessary treatments or products. We have an in-office dental laboratory with a master dental ceramist to create customized crowns, veneers and full-mouth restorations. We use quality materials that are highly esthetic and have been well documented and researched. We use dental implants and parts that demonstrate excellent success rates. We will spend as much time as needed to plan your case to achieve the result you are looking for.

Many patients with old dental implants need new implant restorations with a new design. These patients often present with failing teeth that need to be extracted and the existing implants can used and connected to support a new implant bridge. Sometimes more implants need to be placed to help support the new bridge.

In this case, failing teeth were extrtacted, failing implants were removed and the healthy existing implants were used to support new zirconia bridges to replace all of the teeth in the upper and lower arches. This treatment not only improves oral health, but also improves function and greatly enhances the esthetic appearance of his smile.

Are zirconia dental implants a healthier option compared to titanium implants? This is now a very common question with our patients. Titanium implants have been trusted for decades for their strength, biocompatibility and proven success. However, many patients are now seeking a metal-free option and are requesting more information about zirconia implants as an alternative.

In this in-depth guide, Dr. Adam Geach, Harvard-trained Prosthodontist and Diplomate of the American Board of Prosthodontics, explains the key health benefits, potential risks, durability, and aesthetic differences between these two implant materials.

💡 Whether you’re considering your first dental implant or exploring a replacement, this article will help you make an informed, confident decision for your oral health.

Here is another case with an old ceramic bridge with exposed margins, see arrows above. #7 was crowned and a new zirconia bridge was made for #8-X-10. These restorations are always designed with a facial window for porcelain layering. This adds some translucency and lifelike appearance to the restorations.

History of trauma to #8 resulted in extraction and implant placement. A large, discolored composite bonding on #9 was removed and restored with a veneer. It’s always difficult to precisely match the different materials and block-out underlying shades. #8 crown is zirconia to block-out the custom titanium abutment and #8 is lithium disilicate. Small adjustments to the midline and tooth proportions makes these cases even more difficult.

We could have crowned #9 instead of the more conservative veneer restoration. Ultimately it depends on the lab and communication with the patient.

This patient was referred to me for replacement of the upper right 2 implant crowns. You can see the significant soft tissue loss and the exposed metal of the implant crown in the top photo. These 2 implant crowns were removed and redesigned. Custom titanium abutments and splinted zirconia crowns were made for these implants. Splinting was necessary to add pink porcelain in attempt to mimic the natural contours of the teeth. Thank you

Sometimes impacted wisdom teeth can create a defect at the distal surfaces of the second molars. These defects are deep, subgingival and difficult to restore. I find that the best long-term restorations for these defects are gold inlay or onlays. Gold is such a great material for second molars becasue it can be hidden, it’s a non-brittle material and will last for many years. This is a much better restoration compared to composite resin which will stain and leak over time.
