IW Care Connection, Inc. Established in 2001 under the name of Care Connection, Inc. In January 2011, the organization changed its officers and name to iw Care Connection, Inc.

We are an independent company which provides Nurse Case Managers to assist with cases in a non-biased manner. Our registered nurses work with insurance companies, applicant and defense attorneys, and Third Party Administrators. The Nurse Case Manager coordinates and manages medical and non-medical needs for the injured worker then reports back to all involved parties. Case management services with iw Care Connection, Inc include:

- Comprehensive home assessments to assess in-home care needs
- Catastrophic care management
- Facilitation and coordination of medical care for accepted diagnoses
- Attendance at physician appointments as deemed necessary
- Telephonic case management if preferred by the parties
- Discussion with physicians for clarification of medical issues and reports
- Discharge planning after hospital or skilled nursing facility discharge
- Arrangement for authorized treatment, transportation, and durable medical equipment
- Locate facilities and oversee home health care agencies
- Report all case issues to the adjuster and attorneys
