A 15-minute visit isn’t always enough. 💙

Enhanced Case Management (ECM) is helping our most medically complex patients get the support they need between visits — from housing and transportation to medication coordination and consistent follow-up.

This is what it looks like when care becomes proactive, not reactive. When patients feel seen. When health outcomes truly improve.

Colorectal cancer is now the leading cause of cancer-related death among men and women under age 50 — but the good news is that early detection can save lives.

Thanks to a $15,000 grant from California Colorectal Cancer Coalition - C4, Via Care can offer Cologuard®️, a non-invasive, at-home colorectal cancer screening option to help our community take charge of their health.

This simple test allows patients to screen for colorectal cancer from the comfort and privacy of home — no preparation or in-clinic procedure required.

Talk to your provider to see if screening is right for you. Your health matters, and early screening can make all the difference. 💙

From February 9–12, Via Care Community Health Center proudly participated in National Association of Community Health Centers' 2026 Policy & Issues Forum (P&I) in Washington, D.C., the nation’s premier gathering of Community Health Center leaders and advocates.

Community Health Centers (CHCs) play a vital role in the health of our nation. While they account for just 1% of total annual U.S. health care spending, their integrated, comprehensive care model improves outcomes, reduces costly emergency room visits, and delivers significant savings to taxpayers. Today, CHCs serve 1 in 7 Americans — nearly 52 million people nationwide.

Their impact extends far beyond health care. CHCs have saved Medicaid an estimated $58 billion, support more than 800,000 jobs, and generate over $26 billion in economic output across the country.

Via Care was honored to stand alongside fellow health centers, advocating for the communities we serve and reaffirming our commitment to providing high-quality care for all. A special thank you to:

⭐️ U.S. Representative Judy Chu

Did you know pets can support both your mental and physical health? 🐾💙

Research shows that pets encourage movement, reduce stress, and foster connection at every stage of life.

In fact, UC Davis reports that 60% of dog owners meet weekly exercise goals, thanks to about 150 minutes of moderate or 75 minutes of vigorous activity. Your furry friend keeps your heart healthy… literally!

And it’s not just physical health. Studies show that older adults with pets may experience slower cognitive decline, especially in memory and verbal skills.

This Valentine’s season, we’re celebrating the beloved pet members of the Via Care family, the companions who support our health, brighten our days, and bring our community closer together.

📸 Drop a photo of your pet in the comments and share the love!

🚨 Measles Update in LA County

LA County has confirmed multiple measles cases in the past week. Measles is highly contagious, and early awareness is key. Symptoms may appear 7–14 days after exposure and can include high fever, cough, runny nose, red or watery eyes, followed by tiny white spots inside the mouth and a red rash that starts on the face and spreads downward.

The MMR vaccine is the most effective way to prevent measles and protect yourself, your family, and our community.

Stay informed through Los Angeles County Department of Public Health, and if you have questions about symptoms or vaccines, our care team is here to help.
