Update, because it's been a little bit.

Most recent CF clinic visit was great. His FEV1 (lung function main number) was 89% which is the highest it's been since he started on Trikafta, the original modulator that he was on. He continues to culture nothing but normal staph on his sputum cultures.

His metabolic panel has been close to perfect. This is something that has to be monitored regularly when one is on a modulator. The Alyftrek does not seem to be affecting his liver, is what this means. Yay. Alyftrek is also continuing to keep him in great health. It's been better than Trikafta with his gastro stuff, which makes it even better than Trikafta.

Yesterday he had an upper endoscopy, which he needs to have regularly because CF liver disease can cause esophageal varices (varicose veins in the esophagus). Happy to report that the varices he had almost 3 years ago have not grown and are, in fact, maybe even better. Woot!

Last week we also did his yearly echocardiogram and cardiology visit. His aneurysm remains the same. He will keep taking carvedilol, which is helpful at keeping the aneurysm small by regulating his blood pressure, and also helpful with the portal (liver main vein) hypertension.

What's next? Just keep on keepin' on. Usually this means fighting with the pharmacy (Walgreens Specialty), which is terrrrible.

Lung function today was 86%, about the same as last time. The infectious disease doctor saw Ricky today and she is inclined to not start any antibiotics since he's doing great and his chest CT scan shows no mycobacterium damage. If he cultures the m. fortuitum or m. chelonae and/or starts to show symptoms, we will evaluate. So yay!

Ricky has cultured a mycobacterium. Last year he cultured a different one but we heard "wait and see." Now they're taking it more seriously. He had a chest CT last night to see how bad the damage is (I can't imagine it's that bad since his lung function has been steady). We don't have the results yet. At CF clinic in a couple of weeks we will also meet the infectious disease doctor. I like ID docs... Dr. Robert Armstrong (RIP) saved my mom's life back in 2000 when she had necrotizing fasciitis. Hopefully at that visit we will formulate a plan. It looks like this plan will likely include months of IV antibiotics. He does have a port so hopefully he can do it all at home.

The liver doctor has given the go ahead for Ricky to switch from Trikafta to Alyftrek, the new modulator! It'll take a while because we need to wait for Medi-Cal to authorize it. But at least the process has been started!

Well, it's been a while since I updated!

Ricky is doing really well! His lung function is in the mid to high 80s consistently when it's checked at clinic.

A new modulator drug was recently approved by the FDA and at his next visit, later this month, we are going to discuss him switching to that one.

He had his twice-yearly abdominal MRI recently, because of his CF liver disease, and to sum it up... The MRI looks pretty good. There was constipation as usual (yay CF). They did find the right kidney (which shriveled up to the size of a prune years ago, limiting what meds he can take) and now the left one is larger to compensate, which is interesting. The spleen is slightly enlarged. Gallbladder missing (has been for a couple of years). Pancreas is almost entirely fatty (I have heard that this happens over time with CF) with some cysts. Liver is about 2% more fatty than normal which is pretty good. A new finding is some benign hemangioma lesions adjacent to four of his lumbar and thoracic vertebra. These are just clumps of blood vessels.

And no cancer! Hooray. Because he has CF liver disease, cancer is always a concern. But so far so good.

Nine years ago he was having his most scary hospitalization and as of last month he hasn't been in the hospital for five years! Yay for Trikafta. (Pic below is from nine years ago.)
