A records request form will be required. We suggest requesting records through your new provider, as a fee will be associated with providing them directly to the patient. Reading through some of the comments on our page, we realize we need to provide some clarification.

First, as stated in our new phone message, we ARE checking voicemail regularly. We ARE returning calls, so, please, if you have a need, leave a message on the 970.669.3298 or 970.622.2015 voicemails--these are secure and password protected. Typically we are able to get back to you within 24 business hours.

Second, we are moving through our records requests in the order received as quickly as we can. I'm sure you can imagine the hundreds of requests we have received, and it takes much longer than a click of a button, as with an electronic record, to copy and prepare paper charts for transfer while maintaining HIPAA compliance. Additionally, many of our faxes are not being accepted by the recipient, complicating things. This causes the need to resend the same record, starting at page one. multiple times. It makes for a very time-consuming process. We are doing our best to streamline it, so please be patient with us. If you prefer to pick up your records (which would be our preference as well), we can certainly arrange pick up times with you.

Third, as stated in our letter, records requests can be sent to:

If this changes, we will make sure it is posted and will forward the PO Box to the necessary address for those that do not see our posts.

Believe us when we say we know this is a difficult transition, and we are very sorry. For any other reason than medical, we could have and would have given our active patients much better notice. This is certainly not the way we would have chosen to do this closure; it is a burden, not only on our patients and their families, but also to the facilities taking our incoming transfers.

Given the suddenness of the news, our letter announcing Dr. Quintana's retirement and the closure of Loveland Youth Clinic has not yet made it to our patients. I understand there is speculation as to whether the clinic will remain open. Unfortunately, it will not. Our last day of clinic is Thursday, 02/09/2023. On Friday 02/10/2023 from 11am to 2pm we will have an open house style farewell here at the clinic.

Below is a copy of the letter you should be receiving shortly.

"Dear Loveland Youth Family Members:

As many of you know, I have been practicing medicine in Loveland since the mid-1970s, and what a wonderful and fulfilling time I have had. It has been absolutely delightful watching the youth of our community grow from infancy to adulthood in those many years. We have the grandchildren and, in some cases, GREAT grandchildren of my very first patients walking through our clinic doors! You, my patients...my family really...have been a true joy in my life.

In those decades of practice, I have grown too, not in height maybe but in years. My physical condition may start to prevent me from providing you the best medical care I can. Due to concerns about my health, my personal physicians have greatly encouraged me to retire immediately. It is not easy to give up something that has been such a pillar and important part of one's life so suddenly, but I know this is the best decision for our well-being.

It is with great sadness and a heavy heart that I am officially announcing my retirement and the closure of The Loveland Youth Clinic, P.C. following our appointments on February 9th, 2023. Our other, very capable providers will be tending your needs Mondays, Thursdays (mornings only), and Fridays until then. The clinic will be closed Tuesdays and Wednesdays. My staff has informed me there is to be a "Q's open house farewell shenanigans shindig" Friday, February 10th from 11am to 2pm, and all are invited.

There are several wonderful pediatric clinics in our area including Banner Health, UCHealth, Thompson River Pediatrics, Sunrise, and The Youth Clinic just to name a few. All these clinics have excellent, trustworthy providers and would be a great choice for your medical needs. Records requests should be sent to PO Box 2439, Loveland, CO 80539.

I want every one of my patients to know you have given me the best possible professional life. You have cheered me up, encouraged me to be a better person, and taught me so many things. My heart is full, but I will miss seeing your smiles and sharing in your enthusiasm for life.

After almost 14 years at Loveland Youth Clinic, Dr. Konda is returning to her previous career in hospital medicine. She has been honored to be a part of so many families' lives and has treasured every minute. The LYC family will miss her greatly and wish her the best of successes. Please welcome our new nurse practitioner, Melanie Mehle.

Melanie is very excited to join the team at Loveland Youth Clinic! She has a strong passion for pediatric medicine, and has spent the majority of her nursing career in the NICU, ensuring the littlest patients get the best start they possibly can. Melanie is an advocate for partnership in care, and loves getting patients and families excited about health and wellness. Melanie is originally from the east coast and has lived in Colorado for the past decade. She enjoys hiking with her dog, reading mystery novels, and finding the best coffee shops in the state.
