DHC is hiring an RN or LVN for Home Health & Hospice (full-time) opportunity available. RN or LVN will be responsible for providing both home health and hospice patient care in the patient’s homes alongside a Case Manager. Applicants must be a RN or LVN with a current license in the state of TX, have current CPR, and have a minimum of 1 year experience as a RN or LVN in the clinical setting. We are hiring for a part-time clerical position. Actual work hours are negotiable but will add up to 15-20 hours per week. Duties would include filing, data entry, and answering phones. DHC is looking for a part time Chaplain. Hours are very flexible. Duties include making scheduled chaplain visits with hospice patients and/or families, attending an interdisciplinary team meeting every other week, performing an initial assessment on newly admitted hospice patients.
