💫 Iliotibial Band Rehab

🦵 The Iliotibial Band (ITB) is a continuation of the tendinous portions of tensor fascia latae and of the lateral gluteals. It travels along the outside of the thigh and attaches to the superior (top) portion of the tibia with remnants connecting to the patella. ITB pain can be found anywhere along its path, but most commonly occurs along the lateral hip and knee regions.

🚴‍♂️ITB Syndrome is common in runners and cyclists due to the continuous bending and straightening of the knee during these activities. In this scenario, it becomes irritated along the lateral (outside) knee due to excessive friction against the lateral femoral condyle. More specifically, the posterior fibers of the ITB impinge against the femoral condyle right after foot strike and at approximately 30 degrees knee flexion.

🏃 In addition, during running activities the lateral hip is subjected to repetitive stress and may result in proximal ITB sensitivity. This is more pronounced with simply muscular fatigue, weakness in the hip abductors, and/or poor dynamic single limb stability.

🏋️‍♀️ It’s important to note that it would require a lot of force for the IT Band itself to be stretched. The ITB is a very thick piece of fascia and would require more load than we can generate to cause any tissue deformation. However a rehab program should include mobilization of the tissues surrounding the IT Band so it can move freely.

🦵 Hamstring Strain Rehab

With Spring and this beautiful weather in full force, I have seen an uptick in running related injuries. One of the more common running/sprinting injuries is a hamstring strain. This can be an aggravating injury to recover from due to the rather slow rate of healing and high reoccurrence rate. 😬

The length of rehab will depend on multiple factors, including the degree of strain and where exactly in the hamstring muscle-tendon unit did the injury occur.

Regardless, in the sub-acute phases there will be an emphasis of re-training the hamstrings to accept concentric and eccentric load. Knowing now that the hamstrings eccentrically slow the leg while running, one can realize how important it is to work on eccentric strengthening. 🏃‍♂️‍➡️

Here are just a couple of many exercises you can do to help prevent or rehab yourself after a strained hamstring. If your pain doesn’t resolve rather quickly, than it would be wise to get evaluated by a local Physical Therapist. 👨‍⚕️

🤾‍♂️ Patellar Tendinitis Rehab Options

🦵 Patellar tendinitis is often referred to as “Jumper’s Knee” because it is commonly found in individuals who participate in repetitive jumping activities. Symptoms of patellar tendinitis include sharp or dull pain in the front of the knee typically along the inferior (bottom) pole of the patella.

🧊 Initial treatment includes a bout of cryotherapy and NSAIDs to control inflammation. The use of a chopat strap can help off load the tendon and allow for greater tolerance to activity. After the acute phase, the quadriceps and patellar tendon need to be progressively loaded.

💪 Often, this can be a nagging injury and become chronic (lasting > 3 months). If so, eccentric strength training can be implemented to promote tendon remodeling and decrease sensitivity. In this case, the LOWERING component of this exercise example should be highlighted.

🔹 Piriformis Syndrome can manifest as localized pain in the glute region as well as nerve symptoms down the leg. Since low back pain can present in a similar pattern, the lumbar spine should be cleared before attention to the piriformis muscle itself.

🔸 Why can piriformis syndrome cause nerve like symptoms? The reason is that the sciatic nerve travels underneath the piriformis in most individuals and actually through the muscle in some (7-21% of the population). A constant or frequent compression of the sciatic nerve can result in irritation and inflammation that manifests as burning, tingling, or numbness like sensations, aka sciatica.

🔹 So, lets say that you have been assessed by a skilled professional and the piriformis has been identified as the cause of the symptoms. How do we treat it? Sure, stretching can help mobilize the muscle but that doesn’t usually fix the underlying problem. It has been found that these individuals present with greater internal rotation and adduction forces with single leg activities. Therefore, a reasonable approach is to strengthen the abductors and external rotators of the hip along with re-education of hip/pelvic kinematics.

🔸 As symptoms subside and strength improves, a gradual return to functional activities can take place with the ultimate goal of returning the patient to previous level of function.

Look at that shoulder range of motion! 👀

Let me tell you a little bit about Terry…

He had to make quite the commute to PT but always showed up early to his appointments and ready to work. 🚘

Halfway through his shoulder rehab, Terry went on vacation. Instead of coming back to PT with lost gains, he returned with more motion because he stayed consistent to his home program. 🫵

When Terry had the mobility and strength to do simple tasks, he wasn’t satisfied. He had the goal of being able to get on the golf course with no restrictions. And guess what…he did. ⛳️

So excited of all the progress Terry made after shoulder surgery! Proud to be his PT.

⚡️ Radial Nerve Sliders for Arm Pain and Numbness

🎯 The aim is to mobilize the peripheral nervous system for a reduction of pain, numbness, and tingling by means of decreasing threat perception.

Conditions in which neurodynamics for the radial nerve can be helpful:

✅ Cervical Radiculopathy
✅ Thoracic Outlet Syndrome
✅ Radial Tunnel Syndrome
✅ Posterior Interosseus Nerve Syndrome

🖐 Above I demonstrate a progression beginning with radial nerve sliders and then tensioners that are options for treatment management. Each variation mobilizes the nerve from neck to fingers; however, the difference is that tensioners place a greater stretch on the nerve.

☝️ NOTE: Make sure to depress the scapula with the unaffected arm prior to performing each mobilization for appropriate technique.

⚠️ It is important that these do not cause a lasting increase in symptoms. If this is the case than intensity and load need to be modified.
