UNIFIED ADULT FAMILY HOME’s (Unified AFH) purpose is to provide a home that captures the unity and love of family. We are committed to encourage each resident to be their very best individual self. Unified AFH ensures the health, safety, dignity, rights and well-being of each resident in a barrier-free environment.
Our home is a spacious, bright, cheerful four (4) -bedroom ranch style home with two (2) full bathrooms, a beautiful living room, large kitchen and dining room as well as a reading and entertainment room that overlooks a large spacious rear yard. Our residents will also enjoy the large deck in the front yard.
A full time live-in caregiver will assist with any residential services needed, including daily living skills, daily activities and safety awareness. The licensee will also reside at the property to address any issue that a resident may request. All staff members have successfully completed the requisite criminal and caregiver background checks and have received initial and annual training as prescribed by the State of Wisconsin statute.
Our staff has provided residential care for individuals of many needs, strengths, and barriers.
The residents that we accept are those of Elderly Advanced Aged, Developmentally Disabled and Mental Health. To ensure compatibility among residents, a thorough pre-admission assessment will be administered prior to residency, to identify the unique needs of the prospective resident.
We have an ADA compliant, handicap accessible bathroom with hand shower option, handrails and ramp access where needed. We provide 24 hour, 7 days per week, year round supervision.
Services offered are, but not limited to Activities of Daily Living (ADLs), transportation to and from appointments, medication management, personal cares, independent living skills, leisure activities (in home and in community), meal preparation and meal planning, as well as providing an all-around healthy home environment are all included for every resident. Our residents will experience if desired, many community outings such as the library, festivals, movies, parks/lakefront activities, and so much more.
Our services focus on empowering each resident to reach his or her maximal level of independence in areas of health, activities of daily living, socialization, recreation, vocation, and quality of life. We will supervise and adhere to all medication requirements, doctor visits, appointments, visitations, outings and weekly home meetings. It is our goal to give each resident a safe, stable, consistent, learning, and comfortable family atmosphere to live in; as well as provide them with the most well trained, patient and compassionate staff in the area.
