Bring the following for LIHEAP applications:
- ID
- Social Security card
- Proof of all income
- Copies of most recent electric bills
- Copy of lease agreement if you live in subsidized housing or your utilities are included in your rent

Personal care services help beneficiaries perform everyday tasks and can be tailored to assist with a beneficiary's unique physical dependency needs. These tasks are called activities of daily living and instrumental activities of daily living.
