I keep people alive for my job. What’s your super power? I’m a Diabetes NP

At the end of the year, I reflect on the gift of life. You don’t think about how valuable your health is until you lose it. That’s what happened to me a few years ago. I became “the patient” - bedridden with Meningitis for almost 6 months. Luckily, I recovered and was left with very minimal nervous system damage and occasional migraines.

The practice of meditation, stillness, and stretching plus massage and sauna seems to be the best adjunctive medicine for me to manage my symptoms. True story, this is what made me become a member of SolSpa, a full service Hotworx studio offering yoga, sauna, and even cool dip (24 hours per day which is key for busy moms or business owners like me).

As an NP for over 15 years, I try to prescribe the least amount of medication necessary and support activities that calm the nervous system and bring joy. In my experience, it’s the time you give yourself which becomes the most impactful.

We know people that feel stigmatized tend to get overlooked. Or diagnosed only after it’s too late.

They sometimes don’t pursue healthcare services aggressively (not bc they’re “noncompliant” )…but to avoid being judged, discriminated, or simply ignored.

Our Key discussion points • SAVE for your next visit!

✅ You’ll need almost 50% more insulin as trimesters progress! But after delivery, those needs drop drastically.
✅ Aim for glucose ranges of 63 to 140 for 70% of your day. (For non-pregnancy, target glucose ranges are 70-180)
✅ Bolusing earlier before a meal, even 30 minutes, can help achieve tighter glucose goals after meals.
✅ Pump settings need frequent adjustments such as: carb ratio, insulin sensitivity, tighter/shorter duration of action, and higher basal rates. So frequent visits or calls are imperative.
✅ Higher insulin needs poses an issue of having to change out pump sites more frequently. So using more concentrated insulins, (like U200) or adding basal tethers can allow for regular pump site changes.
✅ There are no insulin pumps marketed for pregnancy in the U.S, so we adjust settings to meet a woman’s unique needs. Twiist is an insulin pump new to U.S. market with a lower glucose target of 87 (it’ll be interesting to see if this plus other pumps will gain pregnancy approval). CamAPS Fx is a pump system/app available in Europe marketed for pregnancy use.

📣 🗣️Diabetes has CHANGED: This is affecting more adults than kids. That’s why everyone should be screening if there is a question of prediabetes, metabolic disorders, autoimmune conditions, or diabetes family history

Screening involves quick, easy blood tests covered by insurance or free in at-home kits (the reason we do this is now we have medications available to delay or even prevent Type 1 diabetes).

And no sis. I’m talking FDA approved treatment on your insurance plan (no black market counterfeit peptides or random “stem cells” from Florida or Mexico etc 👄👙💰)

“Before Anythang” … bc that’s when we screen 🤌🏼

For autoimmune T1, you just need two positive Aab’s to confirm a diagnosis. After that it’s simple to delay or prevent insulin dependence if you still have beta cell function left.

There are 3 stages of T1D. Which one are you in?
1️⃣ Normoglycemic
2️⃣ Dysglycemic
3️⃣ Hyperglycemic

Welcome to the party pal. So how do we screen for Diabetes? You must differentiate Type 1 from Type 2. Here’s how:

✅Check the blood sugar
✅Trend the glucose
✅T1D Autoantibodies + Cpeptide fasting
✅Oral Glucose tolerance testing
✅ Hgba1c - even a 10% subtle change matters

I found an expired Dexcom G6 Pro in my office cleaning for the New year. After posting on stories, I had so many questions and realized 1/2 of you don’t even know what I’m talking about! So decided to share:

Dexcom announced they are “retiring” G6 next summer; this means the personal AND professional versions. This is (well was, bc I don’t use anymore) a single use G6 applied in the office setting - linking to equipment and receivers belonging to the practice. Unlike the personal G6, it’s a one-time use and has the option of blinding. This was intended to monitor a patients glucose without them seeing it. Blinding vs. Unblinding we know impacts glucose. Sounds crazy but….. 👇

When someone can physically “SEE” their glucose in real time, they begin changing behaviors (food, portions, activity levels etc… and that’s with no medication adjustments!) So we would use this as a “starting point” and compare before and afters. This is still what people are doing in clinical trials.

Fast forward to clinical practice now, I literally slap a cgm on someone’s arm mid-visit any chance I get, and we are unblinded baby … from the moment we start. It was a cool tool back in the day, but like all technology, it’s time to move on, so RIP Dexcom G6 Pro

For reference, billing codes are what we (HCP’s) use for insurance reimbursement: when applying a new cgm and then the interpretation code with a minimum of 72 hours of data. 95249, 95250, and 95251 ✅

How are we going into 2026, with people still not screening for Diabetes ⁉️

Delaying diagnosis leads to higher A1C’s, insulin dependence, hospitalizations, and higher risk for DKA and death. Types of Diabetes don’t “look” a certain way; that’s why it’s important to screen.

Before prescribing, here’s a routine check list that runs through my mind:

✅ Metabolic risk factors or family history
✅ Other Autoimmune diseases
✅ Complete thyroid panel with TPO
✅ T1D Autoantibodies
✅ OGGT and Glucose trending as indicated
What else would you add?

Immunotherapy for Type 1 Diabetes (T1D) stops the immune system from attacking insulin-producing beta cells in the pancreas. This has been FDA approved in the U.S. since 2022, so it’s nothing new.

This targets T-cells or immune pathways, aiming to “re-train” or block destructive cells. Beta cells are preserved, enabling the body to produce its own insulin. independently for longer. Score.
