We are officially in crisis mode. We need to focus on acute care and covid vaccines. We are also doing our best to continue giving care to our Newborns and conduct well visits. We need to prioritize and screen every single encounter. We do not do this lightly. We have no choice. Please be nice to my staff if they call you, when you are on hold, when lines are busy, when you are waiting in your car for a test to be conducted on your child, when you are in the office, when you are awaiting a letter from us.

This holiday vacation was not what most of us planned. Covid cases continue to climb, limiting visits with family and vacation outings. We continue to see increasing demand for visits and testing. Due to employees with home exposures or illness, and the need to keep our office safe, we are working with less staff every day. As we prioritize what is possible for us to do right now, we need to focus our care on those with immediate medical need.
Please realize that we have no choice. Please cooperate with our staff. Our doctors and staff are human beings who are stretching themselves to their limits because we know that our families depend on us.
In addition, we are not able to accommodate all requests for covid tests on healthy children for โ€œback to school.โ€ New NYS testing sites are opening up this week and next, and some schools are making arrangements for testing themselves.
As always, a telemedicine appointment is required to determine the best timing and test based on clinical history.
To allow patients who are ill to get through to us more easily, please do not keep our staff on the line unnecessarily.
The CDC has published new recommendations for Covid illness and Covid exposure with shorter periods of isolation and quarantine. NYS has not adopted these recommendations for the pediatric population and we do not know what the schools will do. We always monitor the recommendations and give you the most accurate information available at the moment.
Please continue to help us stay safe by following office protocols:
One parent per child for all children and infants when possible.
Masks required for everyone over 2 years of age.
We appreciate the trust you place in us. We wish we had better news.

During one of our COVID Vaccine clinics, we discovered how children can so easily help each other overcome fear and anxiety. While one child was waiting in the waiting room, another child comforted her by saying that the shot only hurt for a second. We love what we do in our Pediatric office, and our team is so proud to play a part in the efforts to bring this Pandemic closer to an end.

Dear Parents and Guardians:
We are pleased to announce that we have received our shipment of the Pfizer covid vaccine for children ages 5-11 years old.
We strongly recommend that each child receive the vaccine as soon as possible. We are opening after school time blocks to help make it more convenient to come in.
Please do your part to protect your child, your family and your community and help end this pandemic.

What does a 95% vaccine efficacy mean? Does it mean that if 100 vaccinated people are exposed to covid that 95 will not get it and 5 may come down with it?

This is a common misconception. Vaccine efficacy describes the reduction in risk that comes along with being vaccinated โ€” vaccinated people have a 95% lower risk of getting COVID compared with those that were in the placebo group. It does not mean that 5% of vaccinated people in the trial got COVID. Actually, only about ~0.04% of the vaccinated people in Pfizer and Modernaโ€™s trial got COVID so one could think the efficacy would be 99.96%. Butt not 100% of the people in the placebo group got COVID in the trial either. Letโ€™s say about 1% of people in the placebo group got COVID. The efficacy is calculated as 0.04% divided 1% to give about 95%. Thatโ€™s where the 95% comes from. Not everyone exposed will get infected but vaccinated individuals are 95% less likely to get infected than an unvaccinated person. And for children the 12-15, the vaccine was 100% effective! For comparison, the flu vaccine reduces the risk of the flu by 40-60% any given year (40-60% efficacy) โ€”> people vaccinated against the flu are 40-60% less likely to get the flu than unvaccinated people. The COVID vaccine is one of the most effective vaccines we have.

Information geared to our wonderful teachers:
What is the risk of SARS-CoV-2 transmission to the adult workforce in school settings?
Teachers and other school staff are at the frontlines and must be protected with the same caution as essential healthcare workers. Up to 24% of US educators are estimated to have risk factors for severe COVID-19. Few studies have specifically examined the infection risk to adults in schools. However, evidence suggests that there is greater risk for educators to acquire infection from other adults than children (either in schools or outside of them). In one study of 15 schools and 10 early childcare centers in Australia, the attack rate (secondary cases/contacts) was 1.0% from children-to-staff (1/103) but much higher from staff-to-staff at 4.4% (7/160) [note the attack from child-to-child was very low in this setting, 0.3% (2/649)]. In summary:Protective measures will need to be in place around all children, especially those in middle/high schools, however, protection against transmission between adults in school settings may be even more important. This conclusion should be considered โ€œgoodโ€ news, since we have tools that work for preventing infection between adults and can expect teachers to be better able to maintain these practices than children.

I know that lots of our parents are still concerned with school opening decisions. Here is some Up-to-date info on pediatric studies about children and COVID spread in elementary schools: epidemiologic evidence suggests transmission among younger children is not common. Contact tracing of a symptomatic French 9 year old did not identify any transmission among over 112 contacts. Pre-prints of studies from a region of Northern France demonstrated no clear evidence of in-school spread in primary schools, in contrast to a high (>40% attack rate) transmission in high schools during the same time period.

In summary: Current evidence suggests that while transmission can occur in schools, protective measures can limit spread. Evidence also suggests that children of elementary school ages and younger could be more safely prioritized for in-person learning.
