Third Street Family Health Services Provider of comprehensive health and wellness care that is accessible to all. We improve the health of communities by helping to remove barriers to wellness.

Third Street Family Health Services is a network of non-profit community health centers in northcentral Ohio. We provide behavioral health, community health worker, dental, medical, medication-assisted treatment, OB/GYN, pediatric, pharmacy, and podiatry services. Our network includes:

Third Street Community Clinic, dba Third Street Family Health Services (TSFHS), is a federally-qualified 330-e-funded health center providing comprehensive primary and preventive care. TSFHS was originally incorporated and known as Third Street Community Clinic when it first began serving Richland County in 1994 with one site in Mansfield, Ohio. Today TSFHS continues to provide all its services primarily in Richland, Ashland, and Crawford County-10 locations located in Bucyrus, Ashland, Mansfield, Ontario, and Shelby.

TSFHS goal and overall mission of TSFHS is to continue to provide accessible, quality primary health care to the under-served living within the service area.

TSFHS offers dental, behavioral health, podiatry, psychiatry, OB/GYN, medical, and medication assistant treatment.

TSFHS is Joint Commission accredited and also has earned PCMH recognition. TSFHS also earned 2018 recognition from HRSA for being a National Quality Leader in the categories of Access Enhancer as a Health Disparities Reducer.
