Meet our Ville Platte providers!

Layni Vizena, PTA

Layni was born and raised in Eunice, LA. She graduated with a Bachelors Degree in Biology from LSU in 2017 and completed the Physical Therapist Assistant (PTA) Associate of Applied Science program at Louisiana Christian University in 2023.

Layni is married to Jon-Erick Vizena and they have three kids together: a late son, Callahan James, another little boy, Channing Luke, and currently expecting a baby girl in June 2026.

In her free time she enjoys reading, being with her family, and has recently discovered a love for thrift shopping.

Meet our Ville Platte providers!

Derek Fontenot, DPT

Derek is a native of Ville Platte, LA and an alumnus of Scared Heart High School, class of 2008. He obtained a Bachelor of Science in Kinesiology at LSU in 2012 and went on to receive his Doctorate of Physical Therapy from LSUHSC in Shreveport in 2015.

Derek and his wife Blaire have four children- Everett, Adele, Christian, and Hayden. When he is not at the clinic Derek enjoys spending time with his family, watching his kids play ball, hunting, and all things LSU! Derek provides quality care for general orthopedic/sports injuries and post-operative care.

‼️Meet our newest full time member at our Leesville clinic. Jillian Phillips Williams, DPT‼️

Hello everyone! I’m so excited to be here and honored to be your PT. I can’t wait to walk alongside you on your journey to feeling your best! I’m originally from College Station, Texas, and while I didn’t go to Texas A&M, I headed north to the University of Oklahoma for my undergrad before earning my Doctor of Physical Therapy degree from UT Health Science Center. My husband serves in the U.S. Army and is currently stationed at Fort Polk. When I’m not at work, you can usually find me cooking something new, traveling whenever I can, or spending quality time with my two dogs—who definitely run the house

‼️A New Year🟰A new Quad C clinic #3‼️

New year, new adventures! We have some exciting news to share as we are growing again! We are excited to announce that we now have a third clinic open in Ville Platte, LA.

We are teaming up with Kearstin's former PT school classmate, Derek Fontenot, who will be our clinical director of the new location. We originally hoped to team up with Derek a few years ago when we opened our second location in Leesville, but as it appears, God had other plans! We truly feel this was meant to be, as God has laid out this path for this third location. We had no intention of starting another clinic in Ville Platte, but the pieces fell right into place with Derek in his hometown. Derek and his staff have the same values and work ethic that we strive to have at all of our Quad C locations. This is why we have no doubt that this clinic and staff will continue to prioritize the same core values we have had from the very beginning.
