Physician, heal thyself... or prevent your own injuries

A recent review published in JAMA found that surgeons are at a high risk of sustaining work-related musculoskeletal injuries. It is safe to say the orthopaedic surgeons in my circle and I are not good examples of applying the knowledge we have about acute and chronic overuse injuries to ourselves.

I was asked today about when to use an external fixator. Here’s a fracture I treated last month. Initial films show severe trauma in this pilon fracture. The fracture was reduced and an external fixator placed to allow for provisional stability while awaiting soft tissue to heal from the initial trauma (second picture). Open reduction was performed and internal fixation placed the following week (third picture). Note the prior angulated tibial fracture and pre-existing arthritis that made it a little tougher than the usual! And he picked my favorite cast color AND asked me to sign it ⭐️ These were posted with permission by the patient of course!

Bunion and forefoot correction is one of my FAVORITE 💕 surgeries. Here’s a 6-month follow up of a lovely patient of mine who is so happy with the outcome and pain relief he’s had with this surgery! I use the scarf osteotomy, which is a powerful and stable correction. Note that a bunion is NOT a bump and no bone was resected from the medial eminence! Also note the soft tissue balancing (through tendon transfers and plantar plate repair) that’s keeping the second toe down and BEHAVING ITSELF.
