Patients are then monitored when they are in their usual routines. The platform integrates Information collected through detailed patient assessment, driven by the rules engine, to create a comprehensive care plan. This plan supports patients in making lifestyle changes, and delivers specialist expertise to their clinical care teams, so that they make optimal, clinical decisions for medications and devices. In addition to the technology, Stability Health’s transformative care model supports patients with a human coach they connect with virtually using their preferred technology - phone, e-mail, text, video chat. Patients also have access to specialists to support more intensive education when needed, or requested - occurring at a time that is convenient for them. Medical specialists oversee and ensure care plans, and recommendations are appropriate for each patient. In this way, the entire care team, supported by the platform that standardizes ideal care, provides individualized care to each patient.

Primary care is where the real work of healthcare happens—managing complexity, coordinating care, and building long-term relationships that improve outcomes for patients living with cardiovascular, kidney, and metabolic (CKM) conditions. The physicians we partner with continue to demonstrate what’s possible when primary care is supported with the right clinical resources, data, and care models.

Today is Diabetes Alert Day—a reminder not just of how common diabetes is, but of how poorly we are managing it as a healthcare system.

Despite being one of the highest-spending countries in the world on , the U.S. has experienced rising rates of preventable complications from over the last decade. For millions of patients, access to care does not consistently translate into better health.

This is not a knowledge problem. We know how to manage diabetes. It is a care delivery problem—fragmented support, limited time in primary care, and too little help for patients between visits when most self-management decisions actually happen.

At Stability Health, we’re focused on closing that gap. Stability Health partners with primary care practices to improve outcomes for patients with Cardiovascular-Kidney-Metabolic (CKM) conditions.

Within six months, patients had a 59% reduction in hospital admissions and a 30% reduction in ED visits. We also drive meaningful improvements in glycemic control, while extending the care team with virtual-first support that helps stay more connected to their patients.

Sharon Nisbet, RN, BSN, CPHQ and practice operations leader, has joined Stability Health as Senior Clinical Strategist. She brings extensive knowledge and experience driving meaningful improvements in and financial performance in value-based primary care groups. As Chief Nursing Officer at IMA Medical Group (a full-risk Medicare Advantage primary care group serving a predominantly Hispanic population in central Florida) she implemented the Stability Health program, which led to high seven-figure savings from lower hospital admissions, and less frequent emergency department utilization.

“Sharon will work with prospective clients to identify how they will save money and benefit by partnering with us,” said Peggy Chou MD, President and Chief Medical Officer.

“After a year sabbatical in Thailand, I’m pleased to be back in the world of value-based care,” said Sharon. “I’m particularly excited about joining Stability Health. Stability Health is unique in its approach to diabetes, which is the root cause of many cardiometabolic conditions. It was the only program we found that addressed the needs of patients, primary care , and the physician organizations responsible for outcomes.”

Previously, Sharon was VP, Chief Clinical Officer at Wellcare Medicare Health Plans responsible for care management throughout the organization. She also led the analytics and informatics teams and built WellCare’s platform, which led to improved accuracy and efficiency of the utilization management process.

Sharon earned her bachelor’s degree from the University of Rhode Island and is a Registered Nurse and Certified Professional in .

Not too long ago, Maria Alfaro, a very gifted health coach at Stability Health, shared her thoughts on the how strength training (aka as resistance training) is one of the best tools for managing the development and progression of diabetes.

Here are the highlights.
* Muscle is Medicine. Strength training doesn’t just build muscle - it helps the body use glucose better. When we lift, our muscles open the door for glucose to enter the cells. Over time, that means better insulin sensitivity, and more energy for day-to-day activities.

* How We Train Matters. The volume, intensity and timing of workouts all affect blood glucose differently. Lifting after a big meal can help prevent post meal spikes. Understanding these patterns can inform better decisions for meals, exercise and medication timing.

* Consistency Beats Intensity. Consistent strength training builds muscle, boosts metabolism and improves overall metabolic health, making it one of the best long-term tools for blood sugar control. Even training programs designed for elite athletes don’t progress intensity by more than 10% from one week to the next.
