Prairie Home Hospice and Community Care

Prairie Home Hospice and Community Care Prairie Home Hospice is a nonprofit organization incorporated in 1984 to provide hospice care and services to terminally ill persons and their families.

Hospice is a special concept of care for a person in the latter stages of a terminal illness. Our philosophy affirms life and regards death as a normal process. The focus of care is quality of living and dignity with the patient/family retaining control of their lives and treatments. Hospice cares for the total person with support and comfort for the physical, emotional, spiritual and psychosocial needs of both patient and the family. Hospice care is available to anyone who meets our admission criteria without regards to national origin, handicap, age, race, color, creed or ability to pay.

We are incredibly grateful for Chaplain Barb and the steady, faithful presence she brings to our patients each week.

Her dedication to providing Holy Communion offers comfort, connection, and sacred reassurance to those we serve. Today, she also brought ashes for Ash Wednesday.

Thank you, Chaplain Barb, for your servant heart and for the compassion you pour into our hospice community. You are such a blessing to our patients and families. ✝️

Today, Prairie Home Hospice had the profound honor of witnessing love come full circle. Our Ordained Chaplain and Social Worker, Jen Schmidt, officiated a wedding for a patient and his fiancée — a moment filled with meaning, grace, and deep connection.

“Lyle and Jennifer’s story is not a simple one. They have walked together before, walked apart, and found their way back again. Through the years, through loss and learning, something steady remained between them — what Jennifer calls a timeless love. Today, they honored that love by choosing each other again.”

Marrying the love of his life — a love once pulled apart by the weight of the world — was Lyle’s dying wish. At Prairie Home Hospice, we are committed to honoring what matters most to those we serve, helping make meaningful moments like this possible.

Congratulations, Lyle and Jennifer, on your beautiful wedding day. ❤️
