Sergeant Nate Howard is an American hero.

He was from Marshalltown and served 11 years in the Iowa Army National Guard, where his family said he “loved what he was doing.”

He was tragically killed by an ISIS terrorist in Palmyra, Syria on December 13th while serving with 2nd Brigade Combat Team, 34th Infantry Division.

He leaves behind a loving family and will be remembered as a dedicated soldier and for his love of hunting and video games.

Center Associates recognizes the growing challenge in accessing mental health care for individuals with certain Medicare replacement plans. Due to low reimbursement rates and the limited number of mental health providers, we will, as of Jan 1, 2026, not be credentialed with Medicare United Healthcare plans at CA. We are encouraging Medicare (United Healthcare) patients—during this open enrollment period—to review and consider switching to a replacement plan that offers better coverage for brain health care. Last Minute Cancellation Policy Update
Effective October 1st 2025

NEW POLICY

On the day of your appointment if you call at 8am or after you will be a last-minute cancellation

Three (3) last minute cancellation and/or no shows will result in being placed on walk in

We understand this new change can be difficult, but this new policy will help our provider be able to see patients more effectively and ensure appointments availability for those in need.

• If we are closed, you can leave a message on our answering machine – If we have a message before 8am on our answering machine for your appointment that day – this will NOT be counted as a last-minute cancellation
• If you get placed on walk in, please see our walk in policy

Center Associates has been busy this summer with therapy appointments, medication management, In-Home Family Therapy, Behavioral Health Intervention Services, Community Support Services, and Integrated Health. New patient calls and regular appointments are continuing to occur. Urgent Care (same-day access) continues to meet a daily need in our communities.

Our Tama/Toledo office has been closed due to structural issue that was a safety concern for our patients and our staff. We are working with Tama County to see about other options to continue our service for medication, therapy, and case management.
