Plantar Fasciitis vs. Plantar Fasciosis

Most people — and even many providers — get this wrong.

Heel pain doesn’t always mean inflammation.

👉 The truth?
Many chronic cases are actually degeneration of the plantar fascia, not inflammation.

Let’s break it down:

🔥 Plantar Fasciitis
• Acute condition
• Caused by inflammation
• Often occurs after sudden activity increase
• Usually improves with rest, stretching, and short-term treatment

⚠️ Plantar Fasciosis
• Chronic degeneration of the fascia
• Little to no inflammatory cells present
• Caused by repetitive micro-tears over time
• Requires regenerative treatments, not just anti-inflammatories

📊 Studies show that many patients with heel pain lasting 3–6+ months actually have fasciosis — meaning the tissue is breaking down, not inflamed.

Which means:
