Major Unusual Incidents, or MUIs, are a vital part of protecting the health and safety of the individuals we serve. In this video, the Provider Support Department explains what an MUI is, how incidents are identified, investigated, and resolved, and what steps are taken to prevent them from happening again.

When an unusual incident occurs that could affect someone’s health or welfare, the process begins immediately — from reporting and documentation to interviews and prevention planning. Whether it’s neglect, abuse, or another type of incident, the goal isn’t punishment — it’s understanding what went wrong and working together to make things better.

The team shares insight into how MUIs are handled in partnership with providers, how prevention plans are developed, and how every investigation ultimately serves one purpose: to improve the quality of care for the individuals we support.

This video offers an inside look at how collaboration, accountability, and prevention come together to ensure safety, transparency, and trust in our services.
