McGehee Family Clinic is seeking a dedicated Chronic Care Management (CCM) Nurse to join our team! If you have a passion for patient care, enjoy building relationships, and want to make a meaningful impact in a small rural community, we’d love to hear from you.

Position: CCM Nurse
Status: Full-Time (Monday–Friday)
Location: McGehee Family Clinic – McGehee, AR

✨ What You’ll Do:
• Manage chronic care patients through monthly outreach and follow-up
• Assist providers in coordinating care plans
• Document CCM encounters in eClinicalWorks
• Support quality improvement efforts within the clinic
• Work closely with our team to ensure patients receive consistent, compassionate care

✨ What We’re Looking For:
• LPN, RN, or equivalent experience in chronic care or patient coordination
• Strong communication skills
• Organized, dependable, and patient-focused
• Comfortable working with electronic health records
