2) Click the link in your email to set up your online account.
3) Go to myhealthrecord.com to access your patient portal.

The Patient Portal gives you easy access to:
• Securely message your clinical care team
• Request appointments
• View your health history
• Check your lab results

Note: An email address is required to register, and it must be given in-person at our clinic.

We are restocking our Blessing Closet at Family Health Center on Virginia to help patients who may be going through challenging times. We would love your support!

Items Needed:
• New clothing for infants, toddlers, adolescents, teenagers, and adults
• Toiletries (soap, shampoo, deodorant, toothpaste, etc.)
• New baby items such as bouncy seats, car seats, toddler seats, baby bottles, and more
