Access Your Health Records with Ease!
Getting access to your health records has never been simpler! Here's how:
1️) Give your email address in-person at the front desk.
2) Click the link in your email to set up your online account.
3) Go to myhealthrecord.com to access your patient portal.
The Patient Portal gives you easy access to:
• Securely message your clinical care team
• Request appointments
• View your health history
• Check your lab results
Note: An email address is required to register, and it must be given in-person at our clinic.
