Kybella in the buccal region is an off-label treatment that warrants a little extra chat.
While some clinicians discuss deoxycholic acid for select lower-face adiposity, the buccal region is a high-stakes anatomical neighborhood with meaningful downside if patient selection or depth/plane control is off.
Key clinical considerations:
– Evidence base is limited compared to submental use; outcomes can be variable
– The buccal region sits near critical structures (facial nerve branches, parotid/duct anatomy, facial vessels) thus elevating risk.
– Over-reduction can create premature hollowness, skeletonization, or accentuate jowling, especially as patients age.
– If pursued, this should be limited to highly selected candidates, with explicit off-label consent, conservative planning, and a clear discussion of aesthetic tradeoffs and recovery expectations.
– For many patients, alternative approaches (weight reduction, energy-based tightening, neuromodulator patterns, or surgical consult when indicated) may be the more predictable path.
