My intention is to be a care consultant, someone local who can contact the right people to match the clients unique situation. I’ll listen attentively to what the client's preferences and goals are. Supporting such needs as independent living skills, home safety concerns, medical needs, social activities, and help with record keeping and documents. I will research client eligibility for Medicare, supplemental health insurance, transportation aid, and any Brevard County programs of interest. I am just as delighted to research appropriate avenues to benefit the client's desire to socialize. Perhaps attending church, joining a club, or going to an event. After the initial 2 hour assessment is complete I'll write up a highly individualized Plan of Care using the information gathered. We will talk about the type of services needed, who is qualified to provide the service, how much it will cost, when it can be done, and I'll follow-up on the outcome. I believe the clients participation is a key asset to our success.
