* Require Assistance to Ambulate
*Have Residual Weakness
* Shortness of Breath with Exertion
* Confusion of Cognitive Limitation
* Need the Level of Care provided in a Nursing Facility
* Recipient must be 21 years of age or older with a physical disability that requires assistance to remain at home.

If any of these describe you or your loved one, you may qualify for this FREE program offered by Medicaid.
