Many patients go home with questions, new medications, new instructions, and not knowing when to call if something doesn’t feel right. It can be overwhelming for patients and families, and too often people feel like they’re on their own during recovery.

During the recent winter storm, when travel was unsafe for many patients, Dimer Health experienced a surge in demand. This reinforced a simple truth: care must be built to weather the storm and reach patients wherever they are.

Roads closed, but care stayed open. Through our partnerships with partnering health systems and providers, Dimer Health continued supporting patients at home - answering questions, checking in, and helping guide recovery when in-person visits weren’t possible. This is care designed to stay connected, so patients feel supported no matter the circumstances.

When storms shut down roads, healthcare shouldn’t shut down with them. We must build systems that can weather the storm, ensuring care remains accessible, responsive, and human, no matter what’s happening outside.
