🌸 Perimenopause & Depression — It’s Hormonal, Not Random

During the menopause transition, women face a 2–5× higher risk of depression, even with no prior history.

Why? It’s not just “low hormones” — it’s hormone instability.
Fluctuations in estrogen and progesterone directly impact serotonin, GABA, and stress regulation in the brain.

📊 Evidence:
A 2018 JAMA Psychiatry trial found women on transdermal estradiol + micronized progesterone were ~50% less likely to develop depressive symptoms vs placebo.

💡 Takeaway:
Mood changes in perimenopause are biological, common, and treatable.
You’re not broken — your hormones are shifting, and support is available.

Exhausted, anxious, or losing hair… but your labs are “normal”? 👀
It might be low ferritin — even without anemia.

Common symptoms of low ferritin👇
• Hair loss / shedding
• Fatigue that doesn’t improve with rest
• ADD-like symptoms (poor focus, distractibility)
• Anxiety or palpitations
• Restless legs, especially at night
• Shortness of breath with exercise
• Cold hands & feet
• Brain fog

🚨 Ferritin under ~70 ng/mL can still cause symptoms — especially in women.

How to correct it (properly):
✅ Aim for ferritin >70 ng/mL
✅ Use iron bisglycinate (gentle + well absorbed)
✅ Take in the morning
🚫 Avoid coffee or tea for at least 1 hour
🍊 Pair with vitamin C to maximize absorption

💡 Low ferritin is common in perimenopause, heavy cycles, endurance training, and chronic inflammation.

🧠 Cognitive Efficacy Ranking (Human Evidence)
1️⃣ Magnesium L-Threonate – strongest controlled trial effects
✔️ Up to ~37% improvement in episodic memory vs placebo over 12 weeks (and ~20% executive function gains; ~13% working memory by 6 wks) in adults supplementing 1,500–2,000 mg/day. Estimated cognitive age appeared ~9-10 years younger than placebo.

2️⃣ Creatine 5 g – moderate evidence, energy-driven gains
✔️ Meta-analyses show small but significant improvements in memory performance, processing speed, and attention time vs placebo (moderate effect sizes). Most benefit seen under metabolic stress or in older adults.

3️⃣ Omega-3 (2,000 mg) – modest, mixed results
✔️ Some RCTs find improvements in memory and recall with EPA/DHA ≥1 g/day over ≥26 weeks, but overall evidence is inconsistent with mixed effect sizes. Benefits may be more likely in people with low baseline intake or mild impairment.

4️⃣ Resveratrol 150 mg – weakest consistent clinical support
✔️ Human trials limited and mixed. One reported up to 33% improvement in cognition in older women with 75 mg/day, but overall data are inconsistent/limited for robust effects at 150 mg/day.

Weight gain in perimenopause is not just about calories.
It’s about sleep.

When progesterone begins to decline, one of the first things women lose is deep, restorative sleep. And poor sleep isn’t just exhausting — it’s metabolically disruptive.

Here’s what’s actually happening physiologically:

• Sleep deprivation increases evening cortisol, keeping you in a sympathetic (“fight or flight”) state.
• Elevated cortisol drives central fat storage, especially visceral abdominal fat.
• Poor sleep decreases insulin sensitivity, making it easier to store glucose as fat.
• Ghrelin (your hunger hormone) rises while leptin (your satiety hormone) falls — so cravings increase.
• Inflammatory cytokines like IL-6 and TNF-alpha increase, promoting metabolic dysfunction.
• Melatonin dysregulation further impairs glucose metabolism and mitochondrial efficiency.

So you can eat clean, work out harder, cut carbs — and still gain weight if sleep is broken.

We have to stabilize the nervous system and restore deep sleep first.
That may mean optimizing progesterone, supporting estradiol appropriately, regulating cortisol rhythm, improving blood sugar stability, and addressing micronutrient deficiencies.

You cannot out-diet hormonal sleep disruption.

At Treasure Valley Pain & Hormones in Meridian, Idaho, we take a root-cause, physiology-based approach to perimenopause — not a “just eat less” approach.

If you’re exhausted, wired at night, gaining weight, and not feeling like yourself — it’s not a lack of discipline. It’s biology.

Here’s why your weight loss slowed on a GLP-1 — and what to do about it 👇

If the scale stopped moving, it’s not willpower.
It’s physiology adapting.

🔹 Appetite meds ↓ hunger
🔹 But they don’t stop metabolic slowdown

What actually gets things moving again:

🥩 Eat more protein
→ Preserves muscle
→ Improves satiety
→ Prevents metabolic adaptation

🏋️ Lift weights
→ Muscle = metabolic engine
→ Signals your body to keep burning, not conserving

🚶 Keep daily movement up
(GLP-1s lower hunger and subconscious movement)

📌 Less food alone ≠ more fat loss
📌 Muscle + protein = long-term results
👉THEN discuss changing your medication. The foundation just be right to have a great outcome you can maintain long term.

Save this if you’ve “done everything right” and still stalled 💥

📍At Treasure Valley Pain & Hormones, we focus on body composition, not just weight:
• Hormone optimization
• Metabolic support
• Muscle-preserving weight loss
• GLP-1s done the right way

A knowledgeable provider understands where the fear comes from, but also the current evidence. Today we use mostly bioidentical estradiol (your body’s natural estrogen) and bioidentical micronized progesterone NOT synthetic estrogens and progestins. Research has shown no increased breaded cancer risk, even a REDUCTION of risk when synthetic progestins are not used.

When you’re in perimenopause or menopause and someone asks the loaded question:
“How are you?”

Oh, I’m fine. Totally fine.
Except I’m not sleeping.
I’m gaining weight.
I’m moody and don’t feel like myself.
I’m anxious for absolutely no reason.

But yeah… fine. 😅

✨ Welcome to the menu-posse, 1980s babies.

If this feels a little too relatable, it’s not “just stress” or “getting older.”
It might be your hormones—and you’re not alone 💛

Autoimmunity doesn’t just “happen.” It’s influenced by immune regulation — and nutrients matter. 🧬

🔬 The VITAL randomized controlled trial (~26,000 adults, 5+ years):

• Vitamin D3: 2,000 IU/day
→ ~22% lower risk of developing autoimmune disease
• Omega-3s: 1,000 mg/day
→ ~15% lower risk alone
• Vitamin D + Omega-3 together
→ ~25–30% lower incidence vs placebo
• Benefits strengthened after 2+ years of consistent use

📌 Conditions tracked included rheumatoid arthritis, autoimmune thyroid disease, psoriasis, and others.

💡 Takeaway:
Consistent vitamin D and omega-3 supplementation was associated with a meaningful reduction in new autoimmune diagnoses — not symptom masking, but immune modulation.

Why aren’t we preventing osteoporosis?

✅Osteoporosis doesn’t start at age 70.
It starts in late perimenopause — and this is when women lose bone the fastest.

‼️By the time it’s “diagnosed,” the damage is already done.

Prevention should start earlier and include:
✅Estrogen → the single most powerful hormone for preserving and restoring bone density
✅Testosterone → stimulates osteoblast activity and improves bone strength
✅Weight lifting & impact training → mechanical loading signals bone growth
✅Vitamin D + K2 (together)
 – Vitamin D helps absorb calcium
 – K2 directs calcium into bone and keeps it out of arteries

Vitamin D without K2 = calcium with no GPS.

👉Bone loss in perimenopause is not inevitable — it’s hormonal, and it’s preventable.

If no one talked to you about bone health before menopause… this is your sign.

Follow for hormone-based prevention, not late-stage damage control 🦴✨

🦴 Testosterone & Hip Fracture Risk — What the Data Shows

A matched retrospective analysis looked at men on testosterone replacement therapy (TRT) and their risk of hip fracture.

🔬 Key findings:
• Men on TRT had a significantly lower incidence of hip fractures
• Compared to matched controls not receiving testosterone
• Data pulled from a large national claims database
• Results held after age and comorbidity matching

📉 Why this matters:
• Testosterone plays a key role in bone mineral density
• Supports osteoblast activity & bone remodeling
• Low testosterone = higher fracture risk, especially with aging

🧠 Clinical takeaway:
Optimizing testosterone may support skeletal health, not just energy, libido, and muscle mass.
