I SEE THIS ALL THE TIME.
This patient had a prior blepharoplasty with a non-Oculoplastic surgeon. Too much fat was removed from the left upper eyelid, leaving her with a hollow, or what we call an A-frame deformity.
She also had pre-existing ptosis (a droopy eyelid) that was never addressed.
In her revision, I removed the remaining excess skin, restored lost volume by placing fat back into the left upper eyelid, and performed a ptosis repair.
Unfortunately this is common, I see it even online in before and afters posted by surgeons. The subtle ptosis that was missed, the hollowness, the remaining skin, the pr*****ed lacrimal gland. Eyelid surgery is not just removing skin, it’s all the little nuances that create a balanced look.
