This athletic 30-year-old female came to me with a 4-year history of from and , that had not improved with prior extensive treatment including, physical therapy, medications, chiropractic, acupuncture, trigger point injections, heat, ice, massage, and exercise.

Solution: after taking a deep dive into her lifestyle history, we identified her pain triggers and eliminated them. I did an intradiscal injection, and a epidural. I also targeted her arthritic with PRP. The next step was a structured exercise program, including exercises that would be beneficial for her lifestyle and her scoliosis. I identified and then eliminated the exercises that she was previously doing that were causing more harm than good.

Outcome: At the time of her 8-week follow up she was 98% improved and met her treatment goals of being able to live without daily pain and to return to the peloton bike, Pilates, and rollerblading.

Not all sports doctors are the same.

Many doctors who say they practice “Sports Medicine” are NOT board-certified.

What does it mean to be board certified in Sports Medicine? It means getting accepted to and completing an extremely competitive Sports Medicine fellowship training program, dedicating much of your practice to treating spine and sports injuries, dedicating most of your annual continuing medical education to spine and sports, passing a demanding Sports Medicine exam every 10 years.

Time to do some healing….with PRP.

In this syringe is approximately 12 billion platelets. Recent research supports high-dose PRP (platelet rich plasma).

Unfortunately, most PRP systems in use by health care providers do not generate PRP with high platelet counts. Most patients will end up receiving PRP that will not work that well, if at all or will need multiple treatments to get better.

If you are considering PRP, important questions to ask include: the expected platelet concentration? the volume? the estimated total platelet count? that you will be receiving. Ask how many treatments you will need. When PRP it’s done properly, there is no reason to need series of treatments done within a short time frame.

90-95% of my patients get better with a single injection of high-dose PRP - they don’t need repeat treatments. If they do need repeat treatments, it’s not until years later. Visit my website for more PRP information (link in bio).

Yet, another study supporting higher-dose PRP (platelet rich plasma)!

It is fascinating to see the advances in our knowledge as to how this cutting-edge treatment WORKS when done with proper dosing. Not all PRP is the same.

This review looked randomized-controlled trials (the highest level of evidence) with at least a minimum of 6-month follow up. Studies in which the patients had statistically significant positive response to PRP were separated from those without a significant improvement. The average platelet doses for studies with positive outcomes in the PRP group were compared to those without positive outcomes.

Positive outcomes were seen in studies where patients received platelet doses more than 5 billion platelets. Studies with patients with no significant improvement in outcome had an average platelet those of only 2.2 billion.

This study highlights the variations in the way that PRP is done and why some studies may show a negative response. The dose MATTERS. This is why I now ONLY offer high-dose PRP to ALL my patients for ANY condition, not just arthritis.

- AKA lateral epicondylitis - Most people who have tennis elbow do not play tennis. Furthermore, it is not a tendinitis, but rather a tendinopathy or tendinosis, where the normal collagen architecture is replaced by unhealthy tissue. It’s caused by chronic overuse or misuse by using faulty biomechanics of the common wrist extensor muscles.

- An MRI or ultrasound can help with the diagnosis most of the time, but sometimes these tests are normal. A good sports medicine doctor should be able to diagnose this based on just a history and physical exam.

- Next, fix the biomechanics. This doesn’t mean just doing exercise videos that you find online. It means rehabilitating the entire kinetic chain in the upper body and upper extremity.

- Release any soft tissue restrictions in the area with a therapeutic massage, a Graston tool, or by injecting a little bit of fluid into the area.

- Other common treatments include anti-inflammatory medications, bracing, and steroid “cortisone” injections. These treatments often provide on temporary relief. Steroid injections have been associated with tendon rupture and worsening of the condition.

- If these treatments fail and you need to get back in the game sooner than later, consider a (PRP) platelet rich plasma injection. Honestly, scientific studies show mixed results with this treatment. In my opinion the mixed results are due to 4 factors: a misdiagnosis, failure to address the underlying cause, PRP that has not been properly done, and lack of a proper post injection rehab program.

- In my 15 years of experience in doing PRP injections, when PRP is done properly for tennis elbow, ONE injection should be all you need (not a series of injections that are sometimes recommended).

Interest in medical stem cell therapy is booming. Although the phrase “stem cells”, is often used, the procedure involves grafting a small amount of body tissue that contains stem cells – usually the pelvic bone marrow, which is known as bone marrow aspirate; or from a small amount of fat from the abdomen or flank, which is known as lipoaspirate or autologous fat. The bone marrow or fat can then be injected into the target tissue to heal an injury, to treat arthritis or for cosmetic purposes (facial rejuvenation).

Here are some questions you should ask a doctor before trying these treatments.

• Where will the bone marrow or fat tissue be taken from? The highest concentration of bone marrow stem cells is found in the back part of the pelvic bone. Fatty tissue in the lower abdomen or flank are best for tissue grafting.

• Will the fat or bone marrow be combined with PRP? PRP can be added to enhance the efficacy of the tissue graft.

• Who will be processing the tissue graft once it is extracted? Only a licensed physician should be handling the tissue graft from start to finish.

• What are the available studies for use with my condition and what are the success rates? Your doctor should be upfront with you about this.

• How long have you been doing this type of procedure and what is your success rate? Experience matters of course. See a doctor who has at least 5 years of experience in doing this type of procedure.
