MRD in Early-Stage NSCLC: Breakthrough or Not Yet? 🧬🔬

The conversation around Minimal Residual Disease (MRD) is heating up here at MTTB 2026. As we look at the evolution of ctDNA—from its first detection in 1948 to the clinical validations of today—the big question remains: Is it ready for prime time?

Key Insights from the session:
- Outcome Correlation: MRD positivity is consistently linked to worse clinical outcomes.
- Survival Indicators: Conversely, ctDNA clearance is showing a strong link to improved survival rates.
- The "Actionability" Gap: While the data is powerful, MRD status is not yet considered actionable for making definitive treatment decisions in standard practice.

We are standing on the edge of a major shift in how we monitor lung cancer, but we’re not quite there yet.

💬 Engagement Prompt: If a patient is MRD positive after surgery, would you feel comfortable changing their treatment plan based on that data today? Let’s hear your clinical perspective below.

Today at the MTTB 2026 Conference, we’re diving into the data that is shifting the treatment paradigm for Non-Small Cell Lung Cancer. The integration of perioperative immunotherapy with chemotherapy is showing significant promise in improving patient outcomes.

Key Takeaways from the Discussion:
- Outcome Improvements: Perioperative IO + chemotherapy is consistently raising the bar for survival.
- Stage III Benefit: The most significant clinical benefit is currently being observed in Stage III disease.
- pCR as a Marker: Pathologic Complete Response (pCR) is emerging as an incredibly strong predictor of overall survival.

A special thank you to our expert moderators for leading this session:
👤 David R. Gandara, MD
👤 Gavitt Woodard, MD
👤 Jordan Dozier, MD

💬 Engagement Prompt: How are you currently approaching sequencing in resectable NSCLC? Are you moving toward a perioperative approach for your patients? Let’s discuss in the comments.
