📣 HIRING: PHYSICAL THERAPIST
📍Port Huron, New Baltimore, Harrison Township MI and surrounding cities
We are seeking a compassionate and skilled Physical Therapist to join our home health team, where you will utilize your clinical expertise to assess patient needs, develop personalized treatment plans, and deliver high-quality care to enhance mobility, reduce pain, and promote independence.
Responsibilities
- Provide skilled physical therapy per physician’s plan of care.
- Complete OASIS assessments and contribute to patient goal-setting.
- Implement and adapt therapeutic plans to restore function and relieve pain.
- Observe and document patient responses and update care plans accordingly.
- Educate patients, families, and caregivers on therapy goals and use of equipment.
- Coordinate with interdisciplinary team members to ensure comprehensive care.
- Participate in care conferences and contribute to discharge planning.
- Supervise and mentor Physical Therapy Assistants and Home Health Aides.
- Submit accurate documentation and ensure compliance with agency standards.
- Stay current on best practices, attend in-service trainings, and contribute to quality improvement initiatives.
Qualifications
▪️Graduate of a Physical Therapy program accredited by APTA or equivalent.
▪️Active state licensure as a Physical Therapist.
▪️Minimum 1 year of clinical experience required, preferably in home health.
▪️Valid driver’s license, reliable transportation, and current liability insurance.
▪️Ability to work independently, manage time effectively, and document thoroughly.
▪️Strong interpersonal skills and a passion for serving diverse populations, especially geriatrics.
Preferred Skills
- Familiarity with OASIS documentation
- Knowledge of assistive and durable medical equipment
- Strong team collaboration and communication skills
