Saying the breast is “full of milk” is like saying lungs are full of air. It sounds right, but it is physiologically wrong.

The breast is not a storage bag. It is a living organ with blood vessels, lymphatics, nerves, and milk making tissue. Milk is produced continuously in the alveoli in response to hormones and milk removal. It is not “sitting around” waiting in ducts like liquid in a bottle.

That’s why engorgement is not just milk “backed up in ducts.” Engorgement involves vascular congestion, interstitial edema, and lymphatic compression. The swelling you feel is largely fluid and tissue pressure, not simply just milk volume. This is also why cold therapy works. Cold reduces blood flow and inflammation. If engorgement were only milk sitting in ducts, cold would do nothing. Relief would come only from emptying. But clinically, we know cold decreases pain and swelling even before milk is removed. Milk removal helps, yes. But it helps by reducing pressure and improving circulation and drainage, not because pus or inflammation is being “released.”

Language matters because it shapes care. When we reduce the breast to a container, we miss the biology and we give the wrong solutions. Physiology deserves more than shortcuts. 😮‍💨🤷🏼‍♀️💗

End of 2025 Reflection 🤍

Growth this year didn’t just look like fuller schedules or busier months.
It looked like families seeking support earlier.
Parents asking deeper questions.
Providers reaching out to collaborate—not just refer.

It looked like a growing awareness that regulation, movement, and connection are foundational—not optional.

Every number in this recap represents a family navigating big emotions, feeding concerns, asymmetries, or developmental questions—and choosing a thoughtful, whole-child approach.

I’m deeply grateful for the trust, the collaboration, and the shared commitment to supporting babies and families with intention and evidence-based care.

✨ New Journal Publication! ✨

Huge news for breastfeeding mothers with ADHD — our study on atomoxetine is published!

Our study shows that only very small amounts of atomoxetine enter breast milk. Most mothers had an RID under 0.2%, and even the highest level found was only 0.65%, suggesting very low risk while breastfeeding.

Because of this new evidence, we are updating atomoxetine’s lactation safety rating from L4 to L2, reflecting a much safer profile than previously thought.

A few reports of infant fussiness have been noted, so monitoring is still important. Any concerns should be reported to systems like FAERS to help strengthen safety data.

This research provides encouraging evidence that atomoxetine may be a safe, viable ADHD treatment for breastfeeding mothers.
