Urgent Second Message to Femvida Patients,

Our business will be closed for patient care starting March 13, 2022, however, we will continue
to assist with medical records requests for the next 30 days.

To download your medical records to a pdf file or to take screenshots of pertinent information
please use the Healow App or access your patient portal at healow.com.

For medical records transfer requests please send a message through the patient portal or send
an email to admin@femvida.com. We will email you a medical record transfer form. Once we
receive the completed and signed form we will transfer your records via fax.

If you cannot access the Healow App or log in to the patient portal and you would like to
receive a printed copy of your medical records please send an email request to
admin@femvida.com. We will set up a time for you to come in and pick up your records or
arrange to have them mailed.

We will be checking phone messages periodically so please allow 3 days for a response.

To All Femvida Patients,

We regret to inform you that as of March 13, 2022 we will be closing our practice. In order to assist you during this transition please make requests at your earliest convenience through our patient portal or by calling the office to obtain or transfer your medical records. For patients seen within the past 12 months, requests for prescription refills will be honored based upon clinical appropriateness. Thank you for supporting us in our mission to provide comprehensive health care to the women of our community. It has been a privilege and we will miss you.
