Place of Employment: Client homes, work sites outside of the client home as required and other work sites as required by MED GROUP HOME HEALTH CARE management.
Accountable to MED GROUP HOME HEALTH CARE Director, Administrator, Manager, or RN supervising nurse
Job Summary: The PCW is a representative of MED GROUP HOME HEALTH CARE who provides direct client care. The PCW duties are not limited to those listed as each client has an individualized plan of care. The PCW works under the direction of the company. The PCW will mostly work independently of co-workers or supervisors and therefore must balance good judgment and initiative along with the policies of the company.
Work Hours: This position may require various shifts at various times (including nights, weekends, and sleep-over) in one or more work sites.
Duties: The following list of duties is a guideline of possible duties to be performed independently or as a group of duties in the course of working with the client. The PCW duties are not limited to those listed.
1. Perform tasks assigned by the RN supervisor
2. Document in writing each assignment and turn into the office as directed
3. Promptly report to supervising RN any significant changes in the client’s condition
4. Personal Care
a. Assistance with bathing - sponge, shower ,tub or bed bath
b. Assistance with getting out of bed
c. Turning clients that can’t get out of bed
d. Teeth, mouth, denture and hair care
e. Shaving the client
f. Skin care
g. Nail care
h. Care of eyeglasses and hearing aids
i. Assistance with mobility and ambulation including use of a walker, cane or crutches
j. Changing client’s bed and washing bed linens
k. Client’s laundry
l. Light cleaning in essential areas of the home used during personal care
m. Meal preparation, meal serving and cleaning area used during meal prep
n. Assist with feeding if directed by RN
o. Simple transfers
p. Accompanying client to the MD when prior approval from Med Group RN is given
q. Delegated tasks by the RN
5. Prepares, maintains accurate and timely notes.
6. Maintain client confidentiality of client, personnel and Med Group Home Health Care Operations
7. Participates in ongoing training as required
8. Practices safety by using universal precautions and infection control measures
9. Maintains a positive attitude and neat appearance
Qualifications:
1. Possess successful completion of Med Group Home Health Care ‘s Competency evaluation program.
2. Present a neat appearance and positive work attitude
3. Ability to follow instructions and work independently.
4. Excellent communication and listening skills.
5. Compassionate, patient, and caring nature.
6. Able to work in a stressful environment.
7. Must be physically and mentally fit to handle the demands of care giving.
8. Ability to maintain confidentiality about clients and personnel
General Notes:
The PCW will be required to maintain a safe working environment for themselves and their clients.
There may be emergency situations where the PCW will have to react quickly with sound judgment.
